@milktea13 can’t execute a statutory declaration sa Pinas kasi hindi naman yan yung ginagawa sa atin. A statutory declaration is similar to our affidavit. Another person can say na same lang naman siya iba lang ang terms, he’ll be right but for purposes of clarity at para hindi malito ang reader dapat localised ang info.
There’s no one single form for statutory declarations or affidavits because you can make an oath almost about anything... kung may tinutukoy kang exact na stat dec, please specify kung alin para alam ng magta-try sumagot.
When they say statutory declaration for relationships (or similar statements) yun na yung tinutukoy mo na “notarised” documents. Technically though hindi dapat notarised ang term kasi sobrang generic... anything that a notary public has signed had been notarised no matter what the action it may be... the notary public administers your oaths or affirmations whatever those might be.
And since married na pala kayo hindi mo na kailangan nung other relationship evidences kasi DHA will accept your marriage certificate as evidence of partnership. Did you just want to submit so many evidences?
