@kimpoy angelo will be your middle name then. I’d still put your middle name (mother’s maiden name) under Middle name and just explain your entire name format as to how it’s defined in Philippines on a cover letter when you provide your documents. All the best.

Question po, i have been reading that some members have uploaded their documents for unclaimed work experience during the visa lodging sa immi account while some did not. Will there be any issue po ba kung hindi na ako magattach ng mga documents sa unclaimed work experience? If there will be, will the standard COE be sufficient? TIA

@RheaMARN1171933 hi po. Makikitanong naman po re sa middle name. Kase po nabanggit nyo na it might become an issue kung hindi na declare ang middle name. Na grant na po kame and we never declared our philippine middle name (mother's maiden name) sa application namen. Is there a way po to amend it? Thank you

@corrappyka_CE we did not include beyond 10 years work experience and we did provide supporting documents related to those as well.

@batman thanks for the reply. Ff-up question po, if within 10years but unclaimed work experience you provided supporting documents as well, what documents did you provide? Is it a complete set ulet ng standard coe, payslips etc.? Will there be issue kaya if hindi na ako magprovide? Kc i dont have any supporting documents in terms of the compensation eh. I was not able to keep payslips and remittances kc.

Regarding SSS as proof sa lodging, pano po ung incomplete contribution ko? Nagcontractual po kasi ako ng 2015 and di na nagvoluntary contribute sa SSS.

hello po, question po. nasa part na po kasi ako ng pagupload ng documents. and nababasa ko po dito sa forum na requirement si form 80 and form 1221. however, dun sa course po ng pagfillup ng application, i didnt see any link na would direct me to these forms. required na po ba to do these? or may nababasa din po kasi ako na upon request ni CO chaka sila nagppass ng form 80. thanks!

@kaloidq Dun sa page ng pag-upload ng documents may sariling section ang Form 80 so kailangan sya i-submit.

Hi po.. ask ko lng sa mga VISA 190 for "My Health Declarations"

sino po naka encounter ng pag apply ng new application bago po mag log ng Visa . kilangan ng Health assestment kaso wala po sa list yung Visa 190.

nag create ako ng individual immi account para maka medical po ako before logging ng Visa 190. WALA sa list ang 190

however, same time kami ng wife ko nagsubmit ng documents, pero siya walang notification.

update lang for anyone who might come across my timeline in the future.

positive rin ung ACS results ng wife ko and nakapaglodge na kami ng EOI nung August, anticipating the invite next round.

meanwhile, may pending correction kami for our marriage certificate (read up on RA9048 if you have similar case), sa NOV10 pa babalikan tapos dadalhin sa PSA office for additional 2 weeks processing pa.

also, my wife's passport is still under her maiden name - ultimately, we decided to get her a new passport with her married name nalang to be safe. kumuha na rin siya ng new NBI Clearance pero may HIT, next week babalikan along with her DFA appt.

update again in case somebody might find this useful.

wife got her new passport, (about 10 days ata?) on time naman sila, delivered to our house.

got our invite October round and nakapagupload na kami ng documents - as many as we can provide. our birth certificate, joint bank account, passport and passport photo, diploma, TOR, COEs, all payslips we can provide, stat. dec. from our previous and current manager, PTE scores, ACS results, NBI clearance, forms 80 and 1221, etc. in case you're wondering, some documents are notarized, most aren't.

i ended up not including my middle name (mother's maiden surname) after all - mainly because sa Visitor visa ko before hindi ko siya sinama and i didn't face any problem naman.

pending pa ung corrected marriage certificate namin, kya inattach ko nalang ung erroneous PSA copy with the RA9048 grant letter along with a cover letter to explain, at un muna inupload ko. once makuha ko ung corrected copy, iuupload ko nalang rin.

we lodged our Visa application October 26, currently waiting nalang. bale medicals pa kulang.

Hello po, sa mga naglodge for visa 189, nagkaproblema po sa account ko. Grayed out na po yung upload option, hindi pa po ako tapos. Hindi pa rin po ako nagbayad.

Kapag po ba nagbayad na, makakapag upload ulit ng documents? Or delete ko na lang para create po ng bago.

@zach@052019 makakapag upload ka ulit pag nagbayad ka na. Yun nga lang, di ka na makakapagdelete ng doc pag ganun (in case kelangan mong mag upload ng updated doc).

Di ako sure sa deletion ng immi acct.

Hello, regarding po sa payment. May nakapagbayad na po ba direct sa debit card niyo? Or kailangan talaga credit card ang gamitin? Nag error po kasi sa payment, debit sana option ko.
