@anamarie Hi ana, StatDecs are commonly used in Australia, if you are lodging your visa onshore (in Australia) then you can use this link as reference and have it signed by a barrister, doctor etc (people allowed to sign Stat Decs as per Commonwealth law)

If you are lodging offshore (any other country except Australia), then a written statement detailing your relationship would do. Just make sure that you can prove your relationship. Have it signed by yourself, if you want you can have it notarized as well (you won’t really need to have it notarized).

Have other witnesses write down a statement about you and your spouse’s relationship and let them sign it, you can use that as well.

Hi po..

May mga tanong lng po ako.

Makag bayad na po kme sa DIBP last March (with tax invoice) then sa medical may nakitang scar sa lungs ko. Last December na submit ko na yung after treatment repeat x-ray ko after several requests ng Bupa (normally they will email me after 4 days but until now wala). Then nung chineck ko sa Emedical nka lagay dun "Health case was submitted to DIBP".

Ano po meaning nito? Kung nakapag bayad na ba sure na ba grant ang visa?

di na po pala. kahit po pala kami nlng ang gumawa at magsign. sayang din po kasi bayad sa attorney.

@dyanisabelle From what I understand is ang CO na manghingi nito after sa review nang Medical reports by CO or MOC. Yun din iniisip ko na baka hihingian din kami nito kase 2 ako Xray pati anak ko. Was thinking of getting ready with Health Undertaking just incase they would ask for it, was even thinking of uploading it ahead, pero baka naman i-reject din.. hehehehe

@dyanisabelle wala pong advise na mag submit nung form na un. na CO contact lang kami. So I suggest unahan mo na. wala naman mawawala kung sobra sobra ung requirements na maipasa mo

@Justin @Heprex @Noodles12. At sa iba pa na kakamedical lang. Pa check naman if tama yung e-medical referral letter na naprint ko. See attached file. Kasi I viewed yung sample sa website ng slec iba yung sample nila may logo ng Australia DIBP. Parang eto lang naman ang printable sa My health declaration na portion. I generated our HAP ID before lodging our visa. Thanks

@chococrinkle Ganyan yung referral letter ko nung kumuha ako last year. Not sure though kung may bago. Ang imporante naman ay yung HAP ID at yung required na test for the visa na inaapplyan.

@chococrinkle Dont worry usually may mga forms sa clinic dagdag time nga lang sa pag fillup. ang importante ung Hap ID mo.

@Noodles12. Hi! Yes meron din ako na print na forms ng uniwide to save time na din 3 kasi kami mgpa ma medical this weekend. Si toddler lang ang mag slec for the IGRA. Thank you. :-)

@eynah_gee we used Interswift. Ok naman sila, they collected our stuff on 13-July, shipment left SG 13-Aug (pinuno pa container), arrived in Sydney 17-Aug, delivered to Brisbane the next day. Paid S$805 for 36 cu.ft. shipment, which includes 2 large luggages, 1 massage chair (our prized possession haha), 2 large boxes & 2 small boxes. Plus AU$165 quarantine inspection fee pagdating ng Sydney.
