@wellac kapatid, see below response from BIR. You can try to request from previous employer (if ma accomodate pa nila) or follow below instruction. Di ko lang alam how long will it take pag nag request sa BIR - and very important na malaman mo saang branch nag file ang previous employer mo.

note : hindi ko naituloy na magrequest dito sa RDO kaya I cannot guarantee na positive ang outcome ng ganitong option - pero sharing na din baka sakali makatulong 🙂 call the numbers listed below for a faster response - ang tagal din kasi sumagot ng BIR (1-2 weeks yata inabot) 🙂

In reply, your previous employer will be providing you the BIR form 2316,
that serves as your ITR for the taxable year. If the BIR form 2316 is not
available you may submit a written request to the Revenue District Office
(RDO) where you filed your Income Tax Return (ITR) stating therein the
purpose of your request.
