Hi sis @angelmar17, last aug 14 we lodged our application, tapos unexpectedly nung aug 25 nag early labor ako, yung baby ko premature, super liit but very healthy naman thank God. So nung nagka CO na ako last sept. 20 i informed him na i just gave birth and i want to include the baby im my application. So he asked me to send him a copy of my baby's birth certificate and passport. Di pa ako maka provide that time kasi di pa na release yung live birth ni baby, so i told the CO na were still waiting for the birth certificate to released before we could apply for a passport. So nung nakuha na namin yung live birth ni baby pina CTC namin yun then pina authenticate sa NSO, kailangan kasi ctc yung i.authenticate eh (if di ikaw yung magpapa ctc and authenticate, padalhan mu ng authorization letter and id mo yung gagawa nito). Btw, you can do both within a day lang, ganun ka dali. So the day after pumunta na kami sa dfa dala ang authenticated copy ng birth, nso copy of our marriage cert, passport and a photocopy of passport (bio page and back page) ng kasamang parent. Dapat one of the parents ang kasama ni baby magapply ng passport and during releasing din. After ko mag apply ng passport ni baby i scanned the receipt, with the release date written on it, and the live birth certificate of my baby (colored for both) and sent it to my CO tapos i informed him na i attached the receipt of passport for reference since ang releasing nang passport is on XX pa. So nung nakuha ko na yung passport ni baby (nagdala na rin ako ng passport ko when i claimed it to be sure) i scanned it and sent the CO a copy of it and the live birth. Then i uploaded in eVisa form 1022. After a few days from sending it di kasi nag update yung CO ko so i asked if he needs anything else from our end para mapa medical na si baby and ma add na sa eVisa. Kaya we had a very good morning kanina kasi we received an email from team 6 manager containing my baby's hap id, we dont know what happened to our CO yung manager kasi palagi nagrereply sa amin, so when we checked eVisa ayun na add na nga si baby, di namin alam kung paano yung pag pay kasi they added our baby na di pa kami nag bayad. Bukas magpapa medical na yung baby namin, dadalhin ko yung baby book and yung mga test results ni baby from the hospital nung pinanganak cya and ipapaphotocopy ko, pati na rin ibang requirements ng clinic. Hopefully ma upload agad and magiging ok na talaga yung visa, sana grant na agad.
As for my medical naman sis, nagdala lng ako ng doctor's certificate kasi i have gestational hypertension and my doctor prescribed me a medicine to lower my bp. So yun linagay ni doc na gestational lang talaga. Naka declare kasi yung medicine ko eh. Kaya yung physician ng nhsi asked me to have additional exams, crea and ecg kasi daw baka naka affect sa heart ko at kidney yung meds. Ok naman yung results so far. Nawala na yung link ngayon, thank God.
Sorry po sobrang haba nito π
