Hindi ko po sure kasi hindi ko pa masyado binabasa yun sa health part kasi wala pa po ako dun, pero nabasa ko po na pagpunta mo dapat yata example sa Nationwide, may dadalhin ka form na meron naka-indicate na requested by DIAC yun pagpapa-medical mo. So I guess meron form na kailangan.

What to bring
You must take the following to your health examination:
a valid passport
Note: Where a valid passport cannot be provided, an agreed form of identity verification such as a national identify card and a certified copy of your passport photo page.
your prescription spectacles or contact lenses, if applicable
existing specialist and/or other relevant medical reports for known medical conditions
any previous chest x-rays
If you are completing your health examinations after you have lodged your visa application, you must also bring either:
a health examinations referral letter or Form 26EH and/or the Form 160EH (for online visa applications only, available when lodging your application)
a health examination list provided by your case officer with your paper health examination forms (Forms 26 and/or 160) unless you are going to be completing your health examinations electronically
any other documentation/health client identifier provided by your case officer with your paper health examination forms (see below) unless you are going to be completing your health examinations electronically
Important: You must be able to tell the panel doctor or radiologist and MHS which visa subclass you are applying for.
