Yup although hindi pa namin ginagawa, yun na yun supporting documents for that. Marriage certificate and Birth certificate. Kasi tinanong na namin sa university yun, maiden name daw talaga hindi nila babaguhin to married name. Hehehe.

sa Form 80, at "places ere you have lived", kelangan pa ba indicate each address in the specific country where you lived, if you changed address quite often in that country? In my case for example, I have moved alot here in Singapore, so kelangan ilist lahat? Or pede indicate ko na lang yung latest address?

I think nakalagay dun diba, where you have lived for 12 months or more in the last 10 years. Tama po ba? So you can move alot, like ako meron months na out of the country ako, pero hindi ko na kailangan ilagay since hindi ako tumira dun ng 12 months or more.

Ang purpose naman po yata nun is more on the police clearance. If you lived in 1 country for more than 12 months, kailangan mo kumuha ng police clearance from that country just to assure them na hindi ka law offender. 🙂

@Bryann nope dude I am not referring sa online application. That one you don't need to put a specific address in a country. Sa Form 80, question no. 45, you have to 'list in chronological order, the addresses of the places where you have lived during the last 10 years.' may mga addresses kasi ako here where I stayed only for less than a year, so pano kaya yun..

Hi lifehouse28, yung amin linagay ko lahat ng address since birth, kahit less than 12 months pa. minsan daw kasi hihingi sila sa country ng PCC mo kahit less than 12 months ka nag stay. nabasa ko sa ibang forum. kaya iyun...

Naku kung ganon, mali yung pag-fill up ko ng Form80?! Hahaha! Ang nilagay ko lang dun eh yung address ko dito sa Pinas kse yung mga business trip naman eh short trips lang, dito naman tlga ako naka-base.

Sana mag-comment sila if ever may mga tanong sila sa application ko. Bigla tuloy akong kinabahan 🙁

Teka, ano ba ibig sabihin nung clause na <b>"You must account for every year."</b>? ang pagkakaintindi ko kse nyan eh dapat per year yung basis mo para maging qualify na isulat dun.

@mikai yup yan din ginawa ko. like for my case here in Singapore, all the addresses where I lived kahit 3 or 6 months lang nilagay ko. Sa britishexpats forum ko ata nakita they also included even those backpack hostels where they stayed for a couple of months...

@katlin924 ang pagkakaintindi ko sa "you must account for every year" is you should be able to include an address for every year, no gaps. Not sure ha, di naman kasi ako expert sa english, band 7 lang ako, lol. 🙂

Ayan getting clearer. Every year, means tuloy tuloy yun year.. hindi pwede for example 2001-2003.. tapos 2004 agad - 2008. 😃

@katlin924, para sigurado lang yung akin. hehe yung akin business trip din. minsan nga 2 weeks lang, linagay ko parin dun, naka tatak kasi sa passport yung pag labas pasok ko sa pinas.

Lagay mo lang "permanent" addresses mo with no gaps. Yung mga short stays like vacations, pwede mong ilagay sa "travels" section.

parang ganyan... so regardless kung ano yung mga pinuntahan mong "short stops" dapat may permanent address ka pa din...

If you want to modify form 80, pwede ka siguro magupload ng newer version, then put a note na ignore yung form 80 na nisubmit mo dati because may mga nicorrect kang info.
