Forms 80/1221 and VISA Application

Hello. Newbie in this group.

Just want to ask your experience when filling out Form 80/1221 and applying for the visa:

Anong date format gamit nyo sa forms, eg employment history, travel dates? Abbreviated month ba or yung number. Eg DEC 2020 or 12 2020?
If you are nominated and got accepted for NSW 190, when you apply the visa and submit forms 80/1221, okay lng ba walang ilalagay sa Intended Travel Date to Australia? Or need talaga lagyan?
For NBI clearance, may special request or type ba ito when we book an appointment? Pwede na yung MultiPurpose ang nakalagay as purpose?

Walang specific date format. Either DEC 2020 or 12 2020 keri na yan.
Okay lang na wala.
Multi-purpose na iniisue na clearance ngayon. Yun na gagamitin mo.
