Statutory Declaration (ACS Skills Assessment)

To those who were unable to secure a detailed job description from their previous and current employer, I would like to share with you the Statutory Declaration (Affidavit) format that we used when we applied for ACS Skills Assessment. It goes without saying that this format does not guarantee a positive assessment.

Make sure that you strictly follow the guidelines set out by ACS (at page 14):

@altuser41 - sir tanong lang. itong format kaya na to eh applicable kahit saan? Dito me now kasi sa SG. Salamat po for sharing, now its getting clearer on what should i need to do. Maraming salamat po talaga.

Naku I cannot answer you on that kasi sa Pinas ko lang ginamit yan. Pero kung sa personal opinion ko lang, dapat pwede.

@altuser41, hmmm. ok lang po sir. gumawa ako ng sarili kong thread to ask help from fellow pinoys na nandito SG to give some advice. hope may makaadvice sa akin agad.

after malaman ko kung paano yung process ng stat dec dito sa SG. challenge naman sa akin now kung paano ko papipirmahin yung director ko since hindi naman kasi super close at malamang di ko mapapasama yung para pumirma lang. 🙁

@wolfbomb - am also in SG? How's the stat of your docs gathering? Stat dec, pwede yung atty ang pumirma. Ttwagan n lng ata nila company mo pra iverify yung details na inilagay mo..

@Stoked0419 @wolfbomb dito din kami sa sg. for stat dec, yung 2 previous employers po kasi ni hubby nasa pinas. pipirma po kaya ang lawyer dito?

@maria_sg - hingi ka na lang siguro kahit yung generic coe from those employers.. ganon ang ginawa ko at stat dec na lang din. Then I screenshot the email from the HR saying they have their own coe format that they follow..

@maria_sg. Hehe hindi pa nga din. Wait ko dumating boss ko from Europe next week. Kelangan nya kasi pirmahan yung Coe. Tapos baka sa sabado ako next week pa-ctc tapos upload na din ako docs kay ACS..

@altuser41 Hi! I'm a new member of this site and I am also planning to work in Australia. Pwede mo ba i-post ulet yung sample Statutory Declaration? Hindi na kse sya ma-access e. I'm a former ACN employee and based sa mga nabasa ko here in this site, ACN does not provide a detailed COE with duties and responsibilities. I hope you can help me get a sample para makapagsimula na ko application journey ko. thank you in advance! =)

Guys ang question ko lang, yung Statutory Declaration, di ba yung dati mong lead/manager? Ang pipirma? So ano ang magiging proof na authorized siya and totoo yun since di naman talaga yun galing sa company? Sapat na ba yung notarization? Thanks!

Hello dun sa sample document ng threadstarter, merong section dun sa umpisa related sa Company Name, Company Address and Company Contact Details. Ito ba yung Company ng Manager mo kung saan ka humihingi ng reference?

Yung manager ko kasi wala na rin siya dun sa company kung saan ako nag work. ok lang kaya yun?

@thisisme1 yung sa kin, sa bandang baba ng statutory nka dikit ung business card nung dati nming manager, tpos ung photocopy nun ung ni-sign ng lawyer.
