ray1188 Hi sa mga naguguluhan about stat dec na nasa pinas, affidavit lang na may pirma nung colleague mo then papa notarize, message nyoko if you need template

ray1188 @Kcm will send you the template, and also di ko na siya sinama kasi payag naman yung notary public.
