Search for the latest Form 26 and Form 160 online, fill it out and print it. Pag pumunta ka naman sa clinic, meron sila don, but mag ready ka na para mabilis na. Bring 2 pcs of passport sized photo, with blue or white background. Bring also your old xray films and other medical clearances na meron ka and of course yung fee na 4250.
