Statutory Declaration or Affidavit, does not necessarily need previous manager's or colleague's signature, but it MUST be notarized, and Vetassess explicitly stated that it cannot stand alone which means you definitely need supporting documents to support the claims you will put in your StatDec. This is what I did before with positive result.
You will still need to declare the contact information of the said company so I don't know how you can keep this as a "secret" from your previous employer in case Vetassess chose to contact them. The somewhat good news is, Vetassess don't always do this - but it's better to expect this.
