@jmban33 I've been to 3 forums and read a lot of threads so far wala pa akong nakita na nagrequest pa ng iba after 815, usually last talaga 🙂

@mimic hello, nag fiesta ung mga COs sa application heheh. 3 different CO asked for same the document (which is wierd) ung isa was Police check(kasi mali ung naprovide ko), then ung isa was for employment verification hndi daw nila macontact employer ko so i provided a new contact. then last was the 815 form.

Usually wala na, pero I feel you, nakakatakot 2nd CO contact lol kasi another 28 days.

CO ko din si Simon. Minsan hindi nila chinecheck ng mabuti, nirequest nila nung una with my new AFP (nagkaproblem sa name) , PCC ni hubby e naprovide na yun. Akala ko nga may iba pang PCC na kailangan, na takot ako kasi matagal kunin PCC niya.

Grant na niyan next sa inyo ni @iam_juju 🙂

Hello to All,

This is my first time to join in this forum.

I would like to seek advise from you guys. Our Visa has just been granted this week and acccording to the Grant notification, we need to make first entry on Nov 2016.

Our plan po is to make first entry and tay there for a week then go back to PH. However, my big concern is that i signed form 815 for my daughter and we need to go to BUPA to get an appointment for check-up.

my question is do we really need to go to BUPA even if we're just going to make a first entry? Pwede po kaya na during big move nalang mag appear sa BUPA?

Has anyone tried this? THANK YOU MUCH in advance for you inputs.

I think may nagtanong na niyan ditp sa fprum, not sure which thread 🙂

Hello, thank you for your reply. The instruction is not clear po. Sa form 815, it say's lng na you are required to contact Health undertaking Service when you arrive in Australia ... within 4 weeks of your arrival.

Yung first entry po namin kasi 1 week lng sana kami stay sa Australia. Baka hindi namin ma comply ito.

@krsty_pt oo nga hindi clear. Try mo tawag sa immigration. Sila ang best na makakasagot diyan. Although 1 week is not enough para makapaghealth undertaking. May booking pa ata dun.

@krsty_pt I suggest you prioritize this on your first landing/arrival to Australia. We also had the same status request for our youngest son. Normally, free ang hospital sa pag check up at no cost involved for sure as long as PR kayo. Mabilis lang naman ito magawa kasi X-ray lang ang gagawin. It won't take much of your time. Ask for a doctor's signed check-up document and send this immediately to DIBP. In our case, I will contact our migration agent to do this for us para less hassle. December 2016 pa dating ng family ko doon as per IED namin sa visa grant notice. Mauuna lang ako solo this October 2016.

You can email bupa po and tell them you are flying to AU just to activate your 189 visa for a short period of time lang. They will allow you to make the actual appointment kpag permanent stay na kau dito sa AU. you can email them here hus@bupamvs.com.au

@krsty_pt same dillemma! We will do our firsy entry in nov but will.only stay for a week. Someone told me to go to BUPA if were staying already in australia permanently pero pag just to satisfy the initial entry kahit wag muna. Let me know po if nakaemail na kayo sa dibp and ano sabi

Hi guys! Okay lang ba magpasa ng Form 815 agad paglodge ng visa application? Or dapat antayin na hingan ni CO? I had TB kasi before, mga 2008, but wala naman scar na nakikita sa xray na after nag 6 months medication. Feeling ko nga misdiagnosed lang ako pero wala na, nakapagtake nako ng gamot.

Kapag ba may history ng TB, automatic na hihingan nito? Kahit na clear naman yung xray pero nabanggit mo during medical na may history ka? (Hindi pa ako nakapagpamedical.)

@ladymamba24 ako nagpasa ng form 815 paglodge...wala nga ako finding for TB pero nagpasa parin. Kasi sa medical certificate ko for my past surgery may nakalagay kasi na for monitoring kaya nagpasa na lang ako para sure. Di naman nakaaffect sa IED ko and mukhang hindi ko nga need nun form kasi i got one year expiry ng IED. 😃 So i think lalo na sayo na may past illness na TB...magpasa ka na lang ng form 815. Mas okay ng sobra kysa kulang para DG diba. 🙂

@vylette sige magpasa na rin lang ako nung form 815 pag naglodge na ako. Nakakakaba, hindi naman yun magiging ground for refusal siguro ano po, na may history. Cleared naman na ngayon.
