Nag IE kami early this month.

1) We just called Bupa on our arrival and they just updated my record that we already arrived (they will ask for our HAP ID and other personal details).

2) Mention niyo lang na days lang kayo and kung kelan niyo balak bumalik. Then they will advise you to call them again when you get back to settle.

I suggest using the direct bupa contact number for medical visa services: 1300 794 919 (a free call). Wag yung hotline nila kasi andami pang redirection and madami pang tanong. Hehe

gud afternoon po, pwede ko po matanung kung sino po ang humingi ng form 815? un po bang upon completion ng medical? or Case officer n po ang nanghingi sainyo?

Hi, we did not submit this Form 815 on our visa lodgement. The health assessment indicated like no action required.

Both me and my wife are non-medical related professionals and our daughter is just a year and 4 months old.

I checked the form 815 notes and it says that it is only required for those that have risk for TB exposure and other medical related stuff.

Should we still fill-up this form or not necessary as it may just caused some confusion to the CO?

@olynski walang definite time... case to case yan. depende sa CO or if nagbago ang naka- assigned na CO sa case mo.

In my case, after submitting form 815, it took almost 2 months after the next CO contact..

Hi everyone. I just lodge my 190 visa application thru Skillselect. For my medical, 20 weeks pregnant ako ngayon and that means di ako pwede for xrays. Registered nurse ako kaya madami for sure na exams to do. Do u think affected ang application ko dahil pregnant ako. Ma deny kaya or di ito problema? Thanks

Sa mga nsa forum po n ito, automatic po b pag ng fail sa igRA blood test for TB ang bata ang nag xray magkkroon b ng form 815?

@Pixiepie mag fill up ka na lang din ng 815 baka hanapin din yun sa future ma delayed pa aproval nyo. Pagdating naman ng Oz evaluate naman ng mga dictors un kung kelangan mo pa magpunta ng clinic or hindi na. Cheers

@shrinx29 Salamat sa reply, finally may sumagot 🙂 Nag email me now kay agent para magawa yunh form 815. Meron din nga po pala na note sa medical ko na "no action required". For all of us po yan. Ok daw po ibig sabihin nyan? Does it mean we dont need n ng 815?

@patotoy update lng for all, CO contact kmi.. Form 815 after 3 months.. Kahit pla no action required sa system pwede pa rin n mag require ng form 815. No action required doesn't always mean na cleared.

Upon arrival mo dito sa Aussie, let BUPA know and they will ask details from you e.g. Name, address (aussie) and contact number.

i-send nila yung letter dun sa address mo nakalagay dun sa letter yung details ng hospital and kung anong date ka pupunta.

Sa hospital, sa case ko, sinabi sa akin yung purpose ng 815.

So just to check lang yung nakitang findings sa previous xray ko while outside aussie pa ako.

At pinapili ako kung mag xray ba ako ulit or skin test.

and then its negative.

They have to send sa DIBP.
