I recieved an s56 last week for medical. Its very confusing kasi I recieved 3 s56 correspondence all related sa medical.
1st correspondonce only contains My name, and my 2 kids names age 15 and 2. Walang details sa request checklist kung anu yung need sa medical test.
2nd correspondence now contains my Name, and 2kids and some details about the test needed pero di complete. I also noticed it mentioned na may nakaattach daw na FORM 884 and that outlines which further information is required pero wala nmn nakaattach na FORM 884.
3rd correspondence now contains only the names of my 2 kids with the complete details of the checklist for medical test including the HAP ID.
These correspondence were sent the same day. Not sure if mali lng yung 1st and 2nd correspondence dahil kulang2 mga details.
Nakapagmedical na yung 2 kids ko last week and cleared na yung 1 the other 1 is in progress pa yung status. I'm worried about the FORM 884. Wala kc nakaattached na form 884 di ko alam kung para sakin yun or yun na mismo yung medical request for my 2 kids.
Yung status ko nmn as main applicant wala nmn nabago no action required pa din nmn.
Anyone nakaexperience ng ganito? Not familiar sa FORM 884 (Opinion of a Medical Officer of the Commonwealth).
