Magtanong lang po ako regarding sa COE, nangyari din po sa akin kasi na ung employer nagalit sa akin, in effect ayaw din ako bigyan ng COE, and worse, lahat ng co-worker ko pinagbawalan na bigyan ako ng help like recommendation letters, or anything to testify for me. Ngayon po ang plan ko is I will make Self-Statutory Declaration, with the following supporting documents:
Bank Statement
Company ID
Payslips (even though I am the preparer si I assume weak evidence siya)
Employment Contract
Form 2316 (ITR, again, ako ang nkapirma on behalf of my boss)
BIR Form 1604CF (again, ako ang nkapirma on behalf of my boss, even though may attachment ne BIR acknolwedgement receipt from email), with alphalist
SSS, PHIC and HDMF Contributions List (to prove na nging employers ko tlga sila, PHIC and HDMF still pending)
Resignation Letter
Summary of Compensation and Allowances (self-made reconciliation since ung amount per bank statement n nka-reflect is may kasamang allowance, so kelangan ma-trace ung part ng salary per payslip)
When I inquired sa notary (ung sa Cityland sa likod ng PBCom), usually daw ang stat dec is prepared by someone and usually daw co-worker. I am having issues with this as I said, mhihirapan ako mg-ask ng help kht sa former co-workers ko.

I am not sure if things has changed but 5 years ago my wife issued herself a statutory declaration (or affidavit ata as it is know in PH) that list down her job description, responsibility, etc... In the doc she mentioned that the reason she cannot get COE from company nor get any statement from previous manager.
