Form 80 instructions
● Form 80 must be completed
● Ensure that there are no gaps in dates for any of the following:
❍ Full details of all residential addresses for every month of every year for the last 10 years - do not forget to add your current address. Note: If you are applying for a refugee/humanitarian visa, you must provide all addresses for the last 30 years.
❍ All current and previous passport details.
❍ Full details of all employment - if you have been unemployed for any period, write 'unemployed' and state the dates of unemployment, how you were financially supported and how you otherwise occupied your time.
❍ All educational history including primary, secondary and tertiary education - include the dates, name and address of institution, country and name of course for each course you are attending, have completed or have withdrawn from. You must indicate all activity during the period between the completion of your studies and your examination.
● When providing details of family members, include:
❍ family name, given names and any other names
❍ date of birth
❍ place of birth (suburb/residential area, town/city, state/province/region, country)
❍ sex
❍ marital status
❍ citizenship/nationality details
❍ country where they are living now (if not living state 'deceased').
● Provide details of your proposed arrival date in Australia - you must nominate a city and a state or territory.
● State the purpose of your journey and your intended residential address in Australia you must nominate a city and a state or territory.
● List all International travel you have undertaken in the last 10 years. Note: If you are applying for a refugee/humanitarian visa, you must provide all visits to any other country other than Australia for the last 30 years.
● Avoid the use of N/A.
● State dates in the format DD/MM/YYYY.
