AGOS 40 FORM 2017

Hello everyone! Who among you are currently completing or has completed the new AGOS 40 form (January 2017 release) for AHPRA as a requirement for bridging program? Can you please message me. I need to clarify a few details. I'm going to submit mine next week. Thank you very much!

hello @sunshineee28 🙂 thank you for replying. My sister did call ahpra RE category A identification. What to present, wala kasi ako ng lahat ng naka specify. thank you.

For Identification yung sinend ko lang was a ctc of my passport and prc ID. Wala naman kasi akong pwedeng icheck sa mga nka.specified doon sa list.

@Gene011089 hi gene! yes. I tracked my docs via dhl, as of yesterday, received na sa ahpra. 🙂

@sunshineee28 hello! Ahpra told my sister to tick yung overseas passport w/au visa, alam na daw nila yun. no need to explain. understandable nman daw na wala pang visa. I sent ctc of my passport, birth cert, prc, marriage cert and name change letter from prc, ayan hehe sinigurado ko na 🙂

Tanong lang po;

Yung certificate of good standing, ang PRC po b mismo dapat magsend sa ApHra?pano po proseso nun.

Saka yung docs from CHED evidence that your program study was accredited..

Then from university, diploma po ba yun?pano pag may masters?

Punta lang po ng PRC and mgrequest ng cert of good standing. Sabihan mo lng na ipapamail mo sa AHPRA then ibigay mo yung address and mgbayad ng courier fee. .

Wala akong docs from CHED na ipinass sa AHPRA.

From university nmn mgrequest ka ng TOR, RLE, DIPLOMA, CERT OF GRADUATION, ETC. @ulan889

@sunshineee28 thanks po..totoo bang magiging 6 mos. N raw ang bridging program...yung friend ko kakatapos lng nya..yun daw balita doon..early next year 6 months n.

Cguro po. My mga schools na po na ngchange ng program nila from 3months naging 6 months na effective next year. meron dn nmn po na 3mos pa rin yung inooffer nila na program. @ulan889 ang huling napagtanongan ko na school ay 3mos pa rin naman.

@sunshineee28 wat skul po?kasi nagsesearch ako online halos lahat nakalagay 3 months pero for C.Y. 2017..

@ulan889 ACN po. feb 2018 na ag available sched nila and 11weeks duration pa nmn daw. 5weeks theory and 6weeks clinical

Hi guys ask ko lang nung kinuha nio ung Tor sa school my prinesent ba kau docs from ahpra. Cant seem to find it on the website. thanks guys
