The basic things you need to have are:
Valid Passport - ideally valid throughout the duration of your grad visa Health/Medical Exam - book this as early as possible. not sure how things are now, but back in July/August 2020, the earliest schedule I could get was 2 months away, and it wasn't even the closest Bupa center to my location.
Skills Assessment - only if you are applying for Graduate Work Visa Stream (no need for this if Post-Study Work Stream) Health Insurance - you must have health insurance cover for the duration of the grad visa. I believe this is called Overseas Visitors Health Cover.
Australian Federal Police Check English Proficiency Test - IELTS. not sure if they accept anything else.
These are the requirements for a single applicant. Addtional documents need to be submitted if you have dependents.
I really recommend you book the medical and English proficiency exams right away, as the earliest schedule can be months away. You only have 6 months to apply for the 485 visa (I'm not sure if this has changed due to COVID) from the time you received your completion letter (not your graduation ceremony or the day you receive your diploma). And you need to ensure that your student visa is still valid by the time you lodge your 485 visa application. Once you've lodged your next visa, you will automatically be issued a Bridging Visa A that comes into effect when your student visa expires.
