FORM 80 Questions

Hi everyone, we need your help regarding some of the questions in FORM 80. My wife and I applied for Skilled Independent and the case officer has already been assigned to our application. She requested us to accomplish FORM 80 and we are not sure of what to answer in some of the questions.

Question 22 : Do you have personal contacts in Australia (including business contacts/sponsor)? - Do we need to include our relatives even thou we did not apply for sponsorship?

Question 37 : Details of proposed arrival in Australia - Do we need to fill this out because we are still waiting for our visa to be approved before going there?

Question 46 : List, in chronological order, the addresses of the places where you have lived during the

last 10 years (including Australia). You must account for every month/year. If you are unable

to provide an address for any period of time please provide an explanation. - Do we need to indicate the address for short stays like 3-6 months? What is the minimum number of months that should be included in the application?

Question 50 : Have you undertaken an academic thesis, dissertation or publication (including

completed, withdrawn or continuing theses, dissertations or publications)? - We already forgot the thesis we made during college years and we don't have references anymore. We tried to ask all the people involved but they also forgot. Please advise if we still need to fill this out?

@Aedz : here's what we did sa Form 80 namin dati...

Question 22: di kami naglagay ng kahit sino even though we have friends and relatives. paranoid lang na baka may ipagawa sila or hingin from them na hassle para sa kanila. hehehe

Question 37: naglagay kami ng date kahit di naman sure. approximate lang naman yan. 🙂

Question 46: if you really lived in a place for 3 to 6 months, then yes. for my wife, meron syang short assignments for work in the US for 3 months. we listed it pa rin kasi she really lived and worked there.

Question 50: you should answer "no" to this question. from what i know, di kasama dito yung mga undergrad thesis. so we answered "no" to this question. 🙂

Btw, may mga unclear questions pa kami regarding Form 80.

Question 4 : Did you use the passport/travel document at Question 1

to enter Australia? - Do we need to answer this? Kasi "Did" yung ginamit eh hindi pa kami nakakapag travel sa Australia?

Question 20 : Address in Australia where you can be contacted or will be staying - Hindi pa kami sure kung saan kami mag stay kasi we are planning to move there kapag na approve na ang visa namin. Do we need to answer this question?

Question 39 : What is the main reason for going to Australia? Include details of any dates that are of special significance to your time in Australia. - Inquire narin ako regarding this kung anong best answer. I know it's self explanatory but i just want to make sure na tama isasagot namin. We only have one shot so it's better to ask than to assume.

Question 47 : Give details of all employment including periods of unemployment. - Regarding sa type of business, what are the normal types of business for IT consulting company and in-house development? Can we just put "Consulting - Information Technology" and "Information Technology Services Provider"?

eto hinanap ko pa yung Form 80 namin. haha

we left Questions 4 and 20 blank.

Question 39: nilagay namin "Permanent Residence". that's it.

Question 47: we simply placed "IT".

Ang daming hinihingi ni CO na requirements no? yung PCC ko sa MY eh 2-3 months pa so sabi ni CO aware daw sya sa processing time ng MY kaya i-hold daw muna nya case ko til maprovide ko ung mga requirements that he asked. #gutted

yes.. i used blue pen.. para pagcolored scan, makikita na original handwriting ko yun..

I was just allocated a CO, but she never require me of Form 80... Depende po ba sa CO if mag rerequire or not? Thanks

@meehmooh..yup depende yan sa CO..ksi aq hndi rin hiningan ng form 80 but ung husband q as dependent hiningan xa..

Question po sa mga taga-Singapore about Country of Issue ng Identity documents:

Kung sa Philippine Embassy ba kayo kumuha ng passport dito sa Singapore - ang Country of Issue na nilagay nyo is Singapore?
