form 1221

Hi guys, just a question regarding this form. On my eVisa, one of the required document for my wife (18 years old and above dependent) is "Form 1221 Additional personal particulars form". My question is, is she the one that will fill up this form? On "Part A - Your details" of the form, what details do we need to put here? Is it her (my wife's) details or mine?

@RyanJay: Kung naka-lista po 'yung requirement na Form 1221 under your wife's name, then ang details po ni wifey ang dapat nakalagay sa Part A nung form. ;-)

patanong naman po, as I read above, kung yung form 1221 e lumabas under sa dependent, the Part A should be filled by the dependent. --understood po.

Question po is what about Part B,Part C, and Part D and the rest of the document?

meron kc mga tanong sa form na nakakalito, di ko na alam kung ako or yung defacto partner ko yung tinutukoy. hehe. patulong naman po

Sa amin naman po, yung form 1221 is not required but recommended. Should we still need to submit this? Thanks

Good day guys!

I have a question regarding sa form 1221 po, I am my girlfriends dependent and sa part po na question number 39 and 40 na nag tatanong po kung mag aaral po at mag tatrabaho pag dating sa australia, ang sagot ko po talaga dun is yes kaso po may follow up questions na ina-ask yun details ng pag aaral school at employer, since wala pa po employer at school na mapapasukan, should I just tick the box say's "No" or tick the box of "Yes" then i'll just put on the following up questions "none". Thanks in advance sa response nyo 🙂

What we did on my wife's form 1221 is that we answered "Yes" on the question about if she intend to work in Australia then just leave blank yung mga follow up question tungkol sa employer. Wala naman naging problem.

yung main applicant lang ba ang kelangan ng form 80?

We front-loaded my form 80 and my wife's form 1221, pero nag asked yung CO namin ng form 80 for my wife. So bale, sa akin form 80 lang then kay wife is form 80 and 1221.
