Hi @quesoboy, I can only answer for question 1 🙂
We had our Medical Exam at St Luke's Extension Clinic at Bonifacio Global City nung Monday 6/30. Kami lang (family of 3) yung applicants for Australia and most are Canada. Hence, number 1 kami sa pila 🙂 We went around 2:30pm, then we are all finished by 4:00pm. They have a cutoff of 3pm, so better go there before 3pm.
Below are the requirements:
Referral letter with HAP ID
2 pcs recent passport photo for each applicant
Recent Passport for each applicant
Photocopy of passport (though meron din sila dun photocopier machine, libre lang din 🙂 )
Medical Exam fees
Pagdating mo dun, meron pa din forms na fifill-up-an. Very accommodating ang staff dun so don't worry about, they will guide you all through out kung anong procedure gagawin for each applicant.
