Yup kailangan lahat yan. If the letterhead has the company address, then no need. I think that would suffice. Although, in some of my COEs (not all), I had them put in the country.
<i>"This is to certify that (name) was a full time employee with (company name) located in the Philippines since (start date) to (end date) working a minimum of 40 hours a week."</i>
<i>"Yun 40 hours a week"</i> is not needed kung naka state naman na full time ka. ACS considers anything more than 20 hours (if I recall correctly) a week as full time employment.
