@optic - you can just use any normal paper.. the persons who provided statements for us just signed it, then we attahed their passport/IDs.. that's it.. we actually never sent it to notary/lawyer.. this is just based on my experience... i guess this one is fine, because we got our visa..
The formal stat dec format is only if you are AU , it will be affidavit in most countries..
so if you want, you can create an affidavit, you can use normal paper, isama mo ang magsign sa harap ng lawyer.. and then have it legalized by the lawyer/notary..
Now, each lawyer/notary office has their own format, so ang isa sa mga suggestion dito sa SG daw, eh magdala ka ng usb na meron copy ng statement, ibigay sa lawyer/notary office para sila magcopy paste ng mga statements.. or humingi ka ng format nila..
Tip* - make sure na blue pen ang pang sign nila, para pag scan mo (colored) at submit mo as proof/attachment sa application online, eh makita na original ang signature..
