@jld857 I agree with @gibo43 - maari mong gamitin as reference din ang philhealth at pag ibig contributions. mas ok pa nga sa sa pag ibig kasi talagang naka reflect ang company name. hopefully your contributions from 2014 are reflected na doon. in my opinion, sayang naman ang bayad sa RSEA mo if you won’t push through - mabait naman ang EA kasi tignan mo nagrereply din sila agad.

you can reach out to pag ibig online (may chat services sila) and philhealth. if wala ka log in details madali lang to create as long as alam mo mga membership ID numbers mo, if di mo naman natandaan ang membership ID’s, both can also retrieve for you just give your personal details and employmeny details 🙂 kaya pa yan! @ms_ane sa experience ko, yung pagibig walang company name, number of contributions lang meron. yung philhealth merong company name and kung kelan yung hulog.

Hi @kenroy. pwede ka maglagay ng charts, images, tables, etc as part of visuals ng CE mo. When I did mine, I included a snapshot of the standard we are using in our plant. Naglagay din ako ng process flow diagram, actual pictures, tables, and snapshot ng 3D drawing para mas madaling mavisualize ng assessor yung ginagawa ko. I did not include too much of the technical details (although technical writing sya 🙂) ) para madaling ma-capture ng babasa yung background ko.

@gibo43 @jld857 mas ok nga ang itsura ng summary ng philhealth. Di ko alam kung anong access meron yung nahingan ko ng pag ibig contributions sa philippine embassy dito, pero yung pag ibig summary ko e may employer name at employer ID 🙂

pero kung saan ang mas ok at sure go na @jld857 🙂

@ms_ane kung may screenshot ka nung sa pagibig post mo na lang (burahin mo na lang details). minsan kasi, yung mga nasa government agency hindi nila alam yung mga ganyang details unless pakitaan ng example.

salamat @ms_ane and @gibo43. pwede pa po ba mahabol yun kahit lumabas na yung outcome letter? check ko sa EA website, mukhang need i-appeal and may additional cost yata siya.

yung sa ITR pala wala kayang nahihingi na document sa BIR na tax document galing sa kanila? pumunta ko sa local BIR here, sa employer lang daw. yung meron ako are BIR Form 1604-CF and BIR Form 2316 which are both supplied by the employer.

@jld857 nagtry din ako manghingi sa BIR mismo kaso ganyan din ang response sa akin. may mga kulang na ITRs din ako. tapos, yung company naman, usually latest ITR lang ang minamaintain nila. kaya nagleave ako para kumuha ng pagibig and philhealth records.

Hindi inaaccpet ni EA as proof of employment (secondary documents) ang ITR at SSS? Yan documents pa naman naka ready ko for proof of employment sa Pinas.

@datch29 accepted sir yung SSS. pero yung ITR hindi kasi si employer daw ang nag supply nun, hindi third-party. yung ITR nyo po ba anung form number?

@jld857 ung itr sir binibgyan k dapat ni hr ng copy nyan, bake 2 copy yan uns isa ipapasa sa BIR tpos un isa copy na pirmado bg hr manager. SSS at ITR pinasa ko kay EA pero ung ITR ko is sa present employer ko ung previous company ko SSS contribution po pinasa ko

@gibo43 meron sa pag-ibig yung sa employer. Unang page reflecting employer history, 2nd page yung contributions.

Nakasulat naman doon ay provident dun sa requirments ng EA kaya nagsubmit rin ako ng ganitong requirement kasama yung employment history

SSS - contribution and employment history.

Di ako nagsubmit ng phil health, pero pwede siya as third party evidence.

Pwede rin yung bank statement if meron nagrerelease kung sino ang nagdeposit sa account mo and certification from the bank na payroll yun.

ITR - Form 2306. Should be original importante rin ito. Sinubmit ko ito and consistent dapat yung figures na sinubmit mo sa sss, pagibig, phil health, and payslips. Paginispect niyo ang bir form 2306, dapat magtatally yung figures based sa ibang evidences mo.

Yung SSS meron ako, pero yung Form 2306 wala pa ata need ko i check pag ang tanda ko meron ako nung Form 2316.

@donyx hi sir thanks for replying! Planning to the same hopefully wala na feedback once i submitted. Sir ask ko lang, sa personal engg activity mo lang ba sya ininclude (graphs,charts and stuff)or even ss problems encountered part?
