ACS will not accept personal statutory declaration, dapat third party.
Most of the HR kc, they don't know how to draft your COE as per your requirement, better create a sample and show them. But, if they have duties & responsibilities naman na nafile nila, they can use it. Talk to the HR nicely and if you can convince them,,, e di ok!

I agree, ganun din ginawa ng husband ko. With the previous company ni hubby he gave the information he needed to be included in the COE. Then with his current company he talked muna to his boss about his plan for ACS assessment before he emailed the HR, sabi sa kanya they only provide standard format but we emailed the sample format we need using the COE he got for our travel visa and pumayag naman. Akala kasi nila babaguhin totally yung format kaya most of the time hindi sila pumapayag. When hubby emailed them he provided na agad lahat ng kailangan with duties and resposibilities, then we also send the ACS guideline page para alam nila.
