Not knowledgeable on this... As mentioned, we submitted a self-statutory declaration MEANING it was my wife who signed the document which stated that she was employed in Mang Juan's Inasal so and so and she did this and that during her employment duration from blah to bleh...

Worth noting though that she specifically mention the reason why she was not able to come up with a detailed COE.

sisingit ako para hindi ka malito:

i answered your question regarding the "date" sa previous post.
kung hihingi ka ng employment statement (not from the HR) from your previous manager/superior, <b>that is the affidavit/stat dec.. </b>he/she is attesting to what ever is written on that paper (which is your job desc)... that is why he/she is signing that paper... put his/her name/address/contact info for verification ng CO.. inform them na possibleng tawagan sila...

if you want, you can include his/her company ID/passport to show na he/she is still working on the same company.. mas malakas ang proof na yan..

and then you will need to have that paper notarized/CTCed by a lawyer...

Present company (phils): coe, employment statement, itr, payslip, certificates

Previous 2008 (sg)-10: coe, ES, certificates

Previous 2008 (3 mos-china): coe, payslip. We will try to ask for a stat dec from his co-worker plus his id and contact details.

Previous 2006-2007 (bahrain): appreciation letter, license, cert. We will ask nalang din a co-worker for a stat dec.

Previous 2002-2006 (phils): employment statement, itr, certificates.

Mga sir okay na po ba or may kulang pa? Tapos po yun pag-photocopy ng lahat ng documents kelangan sa A4, b&w lang? Then chka namen ipapa-ctc lahat ng photocopies including diploma, tor, license...

Another question po. If the statutory declaration/employment statement will be coming from an ex-co worker, kung saan po sya presently naka-base dun din po ipapa-notarize? Like for example, an ex-co worker still working in Bahrain and Singapore respectively, so sa Bahrain and Sg nila ipapanotarize yun stat dec na ginawa nila for my husband. Then provide nalang sila ng company IDs nila plus contact details, tama po ba?

Chka pano po kung hindi sa company letterhead nakalagay yung employment statement or stat dec, ano pong dapat gawin?

@Mdy - normally kasi if you want someone to do a statutory declaration kailangan sila mismo mag-execute niyan to say a lawyer. So in response to your question, dapat it should be executed at a place where they can appear.

To add up, statutory declaration is a legal document naman henceforth di kailangan (or should I say di dapat) ito nakalagay sa company letterhead.

Definitely they would have a template for a stat declaration - you can prolly ask them to email you this. Ang mangyayari kasi, si Joyce, the secretary of Andrew will ask you to email your docs a few hours before your appointment. Gusto niya na makita ang ginawa mo na document so that she can ammend if kailangan.

I used to be from SIngapore - i left end of 2012... 🙂

If madami ang ipapa CTC mo, ask for discount... 🙂

parang wala akong nabasa sa ACS reqts na need ang salary information. then when I was trying to mock-submit an application to ACS, parang may nakita ako na no need to submit payslips, ITRs, job contracts, job offers letters, high school records...

sakin naman, yung current company ko lang nagbigay ng salary info. i am thinking ngayon, kung magssubmit ba ko ng payslips and ITRs.

May mas mura po na fee for Stat Dec, Mallal and Namazie ang name sa may Robinson Road. Forty sgd lang ang singil. 🙂

Yaikks im collating my docs, and almost complete! Just one more doc and im ready to have it for ctc, then scan then submit for assesment, one major problem yung mga detailed coe's ko overlapping ang dates! Big issue po ba eto? Ex: last date frm company x : feb 4 , 2008 starting date from company y: jan 31, 2008? Usually kase nagteterminal leave lng ako eh! May bearing po ba ito?

Sa mga nakapag pa assess n po s TRA..accepted po b nila yung Self Statutory Declaration for the Detailed COE? Wala n po kasi available n mag witness n ex colleague ko from my previous work in Taiwan..I'm currently here in Sg..Hope someone can enlighten me..pano mo

Just received my COE from Company #2. Mejo magulo ngayon ang mundo ko. Hehe.

Si Company #2 kasi is an IT Agency. I was hired for a 3-month job deployed kay Customer XX.

Ngayon, I only received a generic COE From Company #2 kasi yun daw ang HR protocol nila. So ok, gagawa nalang ng Stat Dec. Kaso sa ACS, di pwede ang self-dec. Kelangan by another party (tama po ba ako?).

My dilemma is: kay Customer XX ba ako hihingi ng stat dec (Supervisor ko)? Kung may Company Letter Head yung stat dec from Customer XX, hindi ba magkakaproblema yun? Kasi magiging 2 Company Letter head yun - 1 from company #2 and 1 from customer xx.

Magulo po ba? Anyone who can help me on this?

Good day! Meron ba dito sa inyo na hiningan ng payslips besides the COE? I was assigned to a case officer yesterday (YEY! praise the Lord!) but may mga hinihingi syang additional docs at isa nga yung payslips/tax payments from 2004-2011. Eh hindi ko na naitabi ang mga payslips ko ng panahon na yun and pang 4th job ko na din ito. Pwede kaya yun ITR nlng un ibigay ko? anyone of you encountered this? Maraming Salamat! 🙂
