@gerleen_agus oh I see. You're right, magpa-medical ka na nga po kasi may nbi ka na din pala. Wala pa ako CO, 2 weeks pa Lang kasi since I lodged my visa.

@gerleen_agus I just went to the clinic with my TRN and passport. When I got there, they asked me to sign both forms you mentioned. So I think either way, the forms still need to be accomplished. Hope this helps.
