@ktelita if hindi po sia nakalaga sa required documents hindi na kilangan ng Form 1221, kasi it is the same meaning form lng sia sa Form 80. and it is not necessary unless sinabi dun sa application nio po na kilangan nio mag fill up ng form 1221.…

Wala nmn po sa requirements po yung form 1221 po, Hindi po sinabi dun na mag lagay po ng Form 1221 kasi almost same lng sia ng form80. Cheers
