what I did @Bjane was to encode my answers. The only page that I printed and scanned was the signature page then I scanned it afterwards. I used an online pdf editor to insert the scanned signature page back to the pdf file.

@KevinDavidson Yes after mo mag lodge ng visa, lalabas naman ang required documents/health assessments mo. Meron dun na tab for health assessment, so makikita mo dun. God bless!

@KevinDavidson iba yung medical yata ng tourist visa sa medical ng PR. Anyway, mag no-notify naman yan kung anong medical tests ang kailangan mo once you lodge your visa application. May questions dun if you have taken previous medical tests.
