@lily_07 i also went to andrew ee..mura and mabait pa..hehe i called them up and on the same day, nakapagpa ctc ako..kasama ko rin yung nagsign ng stat dec ko. for acs, hindi na ako nagsubmit ng birth certificate..ung copy lang ng passport bio page…

@Ozlaz both forms binigay ng hr saken..ung ir8a during my first year with them (mid year ako nagjoin) tapos ung ir21 naman when i resigned na. i suggest email mo na lang yung previous employer mo..ask if they can give you copies plus isama mo na yun…
