medical for estranged partner

hello po sa admin/seniors patulong po kung sino po meron the same scenario as mine.

i am married (not divorced/annulled) and not intending to include estranged husband (he abandoned me and our son for 2 years already) in the application. is he required to have medical exam as well? is there any way it can be waived?
