@maiSG03 Opo. Ganyan din lumabas sa mga anak ko. Pero ang in-attach ko lang ay evidence of Birth or Age (birth cert), Health Evidence (resibo ng clinic/hospital), Travel Doc (Passport).

Yung sa wife ko hindi ko na din matandaan ng husto kung may lumabas na evidence of custody. Kasi pag sinilip ko yung ImmiAccount ko ngayon, ang lumalabas na lang na attachments ay yung may mga in-attach ako. Yung mga wala ako attachments, hindi na din nag-a-appear.

Anyways, baka hindi mo na din po kailangan ng evidence of custody kasi makikita nila sa birth cert ng anak nyo na kayo ang parents. Or kung may mga school records ang anak nyo na nakalagay na kayo ang parent/guardian. But I guess po, hindi na talaga kailangan yan.

@maiSG03 Sorry po. Hindi ko na din matandaan kung meron sa wife ko ng evidence of custody.

Palagay ko po yung list ng attachments ay parang default lang eh. Kasi yung sa anak kung tutuusin po ay hindi naman kelangan ng Form 1221, English test, etc. Pero nandun din sa list.

@maiSG03 Ah, evidence po iyon sa medical nila. Pero hindi naman po necessary yon. Kasi ako dito sa Saudi nagpa-medical pero wala po akong ganon.

Sa St. Luke's kasi sila nagpa-medical. Nag-email ako sa St Lukes ng proof na tapos na yung medical nila at nag-send sa akin yung St Lukes.

Kung natandaan po ninyo nung nag-generate kayo ng HAP ID, nakalista doon yung mga medical test na gagawin. Sa reply po ng St. Luke's, nakalagay na completed.

Naka-attach po dito para makita ninyo.

@coachella9 yes po. yung my health declaration site is applicable po for applicants na hindi pa nag lolodge.

so for my case po kasi, nag lodge na ko since I've been invited. So I had to undergo the creating the HAP ID after lodging the visa.

Sa philippine passport kasi ang Given Names ay ang First name tapos meron pang Middle Name, nalilito lang ako, kasi nakalagay as per passport. So, as per passport given names is first name lang... tama ba? Sa EOI ko kasi given name nilagay ko first name... wala kaya problem kung babaguhin na may middle name yung first name... thanks... ano ba sa case mo, for referene lang.

@dorbsdee Ganyan din nagawa ko dati. Walang middle name sa EOI. Pero sa visa app sinama ko sa given names (first + middle). Hindi naman nagkaproblema.

guys sorry po mejo OT, ano pong pde i substitute sa COE? Closed na po ksi yung company nmen kaya ayaw nya na mag issue ng COE ska wala po akong naitabi na payslip and contracts since maliit lng na company. Affidavit lng po ba of my boss will do? Ano po kaya need nyang i attach na document sa Affidavit to prove it though nag reflect nman po sa SSS, pagibig and Philhealth yung company? thanks po.

@Supersaiyan statutory declaration pede mo gamitin, tapos papirmahan mo sa boss mo or colleague mo to attest na nag work ka sa company na un, tapos mag submit ka din ng SSS contributions para maipakita na nag work ka nga sa company na un. Note na nagtatawag sila to verify employment details, so make sure na sasagot yung tao na pipirma sa stat dec mo.

@PMPdreamer, thanks po sa insight. Actually, nagbasabasa na po ako sa forum regarding po sa stat dec or affidavit madami n po akong nabasa kaya lang sa sobrang dami nalito tuloy ako kung sino ang gagayahin ko.

Eto po question ko po:

1. it means po na kpag stat dec or affidavit no letterhead na from the closed company since affidavit has own format?
2. need po ba isama si boss sa pagpirma ng stat dec sa notary public or papirmahan ko na po sa kanya tpos procede ako sa notary public?
3. may need po ba sya na iattach to confirm na he has the right to attest me at nag work kmi sa same company?

Guys, another CO contact today. It's our 2nd contact.
She requests for additional documents for the baby that we are expecting this October.

- Form 1022 'Notification of change of circumstances'
- Birth Certificate for newborn baby
- Passport for newborn baby

Has anyone of you been in this situation before? How quickly we can get the NSO Birth Certificate, and what is the process in getting passport for the newly born? Do we need to set-up an appointment, etc? Also, does this mean that our application is somewhat finalized and just waiting for the baby's delivery?

@Chad ilan months na po ba si baby? Based sa FAQ ni NSO:

"Please understand that it may take several months after the date of birth/marriage/death before the birth/marriage/death certificate is received, verified and converted into digital format by the NSO. Therefore kindly allow:
• at least two (2) to four (4) months after the date of birth/marriage/death, if the birth/marriage/death occurred within Metro Manila
• at least six (6) months after the date of birth/marriage/death, if the birth/marriage/death occurred outside Metro Manila before filing your request for a copy of the document."

sa passport naman po, prerequisite yung NSO birth certificate. per experience, pag baby (less than 1yo) no need for appointment, walk in po pwede si baby.
