@jandm Yep, that's right. I was e-mailed twice. The CO indicated in the 2 e-mails were different, but they were both from GSM Adelaide... Did your CO also asked you for Form 815 after you submitted your medicals?

@ram071312, my CO asked me for Form 1221 (Additional personal particulars information) and then, after my medicals, I was also asked for Form 815 (Health Undertaking).
