Lodging Visa Application and Forms 80/1221

Hello po! I got an ITA for 491 VISA for ACT and I'm currently filling up forms and documents. I have filled up most of the forms, and I already have my Police Clearance ready for all the three countries I stayed in (PH/SG/UAE).

I have a few questions though, as medyo kinakabahan ko especially sa Forms 80/1221. Enumerate ko lang po mga tanong.

A. Saang part ng Visa Lodgement i-uupload yung Form 80/1221? Di ko po makita sa upload documents tab.

B. If Form 80 is really needed, do I really need to put literally all the addresses na tinirhan ko for the last 10 years? Kasi a lot of the place I can't remember (I.E. transient place na tinirhan ko sa SG for a month, dating dorms etc). Also, do I need like a proof of residence for those? Hindi ba pwede ilagay ko na lang yung address na pinaka matagal kong tinirhan?

C. Sa Education Part, I graduated from DLS-CSB, pero previously I studied for a year at ADMU and AMA. Should I declared those, and likely ba na hihingan ako ng TOR for those universities na nag withdraw ako?

D. Yung Part H, Proposed Travel Part, ang isasagot ko ba sa "Why are you travelling to Australia?" part is for immigration?

E. May Guide po ba sa pag sagot ng Form 80?

I'm not saying these are correct pero eto mga ginawa ko:

• Just have those ready. Depende pa daw sa officer na maaassign sayo kung hihingan ka ng Form 80. Some uploaded these daw together with their PH character reference (where you upload your NBI clearance). Ganyan na din ginawa ko. Para saan yung 1221?

• Nilagay ko lang yung mga matatagal kong tinirahan na naaalala ko and those addresses that show up in my other submissions i.e. employment contracts, school records, etc. Basta make sure na magtally sila sa ibang submitted documents mo.

• this i'm not sure. Solid UP ako ng sampung taon lol. Pero baka kailangan, I suggest magrequest ka na ng mga TOR and honorable dismissals sa ibang universities na inattendan mo.

• Ganyan ang inilagay ko.

E. May Guide po ba sa pag sagot ng Form 80?

• Wala akong nakita dito pero meron naman online kung matyaga ka makinig sa ibang accent #notracist.

Again, hindi ako agent, hindi rin ako immigration lawyer. Above answers are from what I personally did. Yung ibang mga tanong mo natanong ko na din and nasagot ng matino, yung iba naman natanong ko na din pero walang sumagot so I just answered to the best of my understanding. Hope this helps somehow. Extra question din pala, considered pa rin ba na usual place of residence ang home country mo kahit based na sa ibang bansa (ie currently based ako in UAE)? Or dapat may end date kung kelan huling nag stay sa Pinas at saka nag abroad?

In that case, kelangan ba pati lahat ng uwi ko sa Pinas ilalagay ko sa travel history ng "visited any countries outside of their usual country of residence since turning 16 years of age"?

Im based in Singapore for 10 years. Nilagay ko sa travel history ko ung mga flight na pauwi ng pinas as "Visits back to own country" but in your case, kung di ka umuwi ng pinas within ten years, u dont need to include that. U only need to declare travel history within 10years

I see, umuwi rin ako ng Pinas so kelangan ko na nga rin ilagay. Ideally ba in order ba yung pag lagay ng travel history? Hindi kasi automatic na inaarrange by date

For me, nilagay namin siya in order. Kasi para mas madali sa Immigration officer makita kung may namiss ka bang details. Mas madali nilang basahin, mas may possibility na hindi ka macontact for further information.

Hello, ask ko lang regarding Form 80, is this for the main applicant only? (no need to prepare for spouse?)

Also, need na ba iprepare ung police clearance or wait for CO contact?

Including your spouse. Some prefer to wait until CO contact while others prefer to submit upon lodging.
