another question po para sa form 80... sino po ang hiningian ng CO ng form 80.. then sa instructions na binigay ay kailangan i declare ang primary at secondary education??

If hindi pa tlga ma-clear until matapos un medication nya then most likely i-lodge ko na din today kasi hindi ko mahihintay un June dahil ma-eexpire nman un invitation ko. Sa mga nababasa ko naman dito sa forum, wala naman daw nadedeny pag ganun, nadedelay lang talaga. Pero hoping pa din baka sakali bigyan na din ng grant then mag Health Undertaking na lang, bahala na, dasal na lang muna sa ngaun. Hope anyone who has the same experience here can also share what happened to them.

Un sa inyo, try mo tawagan uli, minsan hindi nila finoforward agad kung di pa i-followup e, un samin nun husband ko after 3 working days cleared na agad sa My Health. Un sa daughter ko tnawagan ko lang on the 6th day tapos ayun saka pa lang sinabi na iforward na nila tapos ganito pa nga kaya lalo na tumagal. Malapit na din yan

Update: Nag-email na sakin un BUPA Medical Services, naka-defer un health assessment nun daughter ko, need daw muna nya tapusin un medication (until June 2017) then after nun, undergo uli ng x-ray then pag cleared na saka pa lang mag-clear un medical nya.

Question naman po, since mag-eexpire un invitation ko ng March 3, ok lang kaya na i-lodge ko na un visa application namin kahit hindi pa cleared un medical ng daughter ko? May case kaya na idedeny nila or hindi lang mabibigyan ng DG since may pending medical pa? Another option na naiisip namin is wag muna cya isama sa application kaso baka mas mahirap pa un process later on. Please help enlighten po. Maraming salamat!

@maren1026 di man DG yan ay magiging grant din yan sa huli. lodge ka bago magexpire ITA. kung gusto mo hintay ka ng mas ok na exchange rate bago maglodge para kahit paano makatipid ng konti.

Wala pong re-issuance ang TRA kaya parang new assessment din pero same ang reference number nya. Yung date is after ma invite kaya medyo worried ako kya inupload ko rin ang luma pra may reference din. Hindi po akong primary applicant nagcclaim lng kmi ng partner points.

@xiaolico initial entry muna kami this July tapos mga May 2018 pa ang big move. Antay antay lang ng grant. Dun din naman ang punta ng lahat

@PCK employment contract etc.. initially kasi wala ko payslip na makita sa first job ko kaya cguro na co contact ako. Anyway buti na lng nakakita ko ng payslip kahit few months lng tpos upload ko na din. Goodluck!

Guys kailangan po ba talaga na magattach ng payslip from previous work during pag lodge ng visa? On my case kasi lahat ng reference letter ko may indicated salary na. My first three company malabo na makakuha ako ng payslip since either close na yung company or nachange na yung payroll system nila. Sa current work lang ako makakakuha any idea ano pang alternative proof na pede gamitin? Ok po ba ang scanned copy ng contracts?

Form 80 instructions
● Form 80 must be completed
● Ensure that there are no gaps in dates for any of the following:
❍ Full details of all residential addresses for every month of every year for the last 10 years - do not forget to add your current address. Note: If you are applying for a refugee/humanitarian visa, you must provide all addresses for the last 30 years.
❍ All current and previous passport details.
❍ Full details of all employment - if you have been unemployed for any period, write 'unemployed' and state the dates of unemployment, how you were financially supported and how you otherwise occupied your time.
❍ All educational history including primary, secondary and tertiary education - include the dates, name and address of institution, country and name of course for each course you are attending, have completed or have withdrawn from. You must indicate all activity during the period between the completion of your studies and your examination.
● When providing details of family members, include:
❍ family name, given names and any other names
❍ date of birth
❍ place of birth (suburb/residential area, town/city, state/province/region, country)

@Superched Pwede rin daw ITR and SSS contributions/employment history based sa mga nabasa ko. Basta maprove lang na paid employment yung kini-claim mo ng points.

@Superched , hinde po kasi enough yung coe na may indicated na salary... ang hinahanap po nila is additional evidence na nagwork ka talaga sa company na yun. pwede po yung payslips, ITR, bank payrolls (statements), or pwede din yung sa SSS website na may list ng contributions per company.

Kung wala na po talaga choice, create statutory declaration nalang po...

@engineer20 yup thanks! Actually nag-lodge nako kagabi, gusto tlga humabol sa january batch hehe (although feb 1 n nklgay na lodge date kasi au time yata) so ayun waiting game na lang at smahan ng dasal

@Superched Statutory declaration dapat gawa ng manager/supervisor mo. Yung iba sila na gumagawa tapos pinapasign na lang sa manager nila. Notarized din dapat.

For those who had their medical exam already - which one is better? St. Lukes or Nationwide? I know standard naman yung exam, but in terms of uploading the results, strict compliance or any other factors. Please share any thoughts. Thanks!

@SAP_Melaka Parehas silang mabilis. St Lukes ako. Si @auitdreamer sa Nationwide - Makati.

Mas pinili ko lang St. Lukes kasi mas malapit samin and ayoko mag drive sa Makati.

@SAP_Melaka Yup parang same lang ang bilis though I noticed sa St Lukes, pag sa babae parang may additional requirement pa sila ng pregnancy test. They also "recommend" MMR. No such requirement for Nationwide Makati.

@baknir parang nakakatakot CO mo hehe. Nagsubmit ka na ba ng form 80 before? Yung lahat ba ng hinihingi sya sayo wala talaga sa form80 mo? Halos lahat naman ng sinabi nya nakaindicate na din sa form 80.
