Documents Confusion (Vetassess) need help!

Hi. I am new here and i have a few questions with regards to the documents that I need to submit. I hope someone can help me. my nominated skill is office manager. I am working for our family owned business since 2003. The business is registered under my parents name until 2011 when it was transferred under my name.

1. nagpacertify ako ng documents kaninang umaga. napansin ko, hindi naka indicate doon ang address and contact number ng law office. sabi nila wala daw sila stamper na ganon. pero may sulat galing sa lawyer nakalagay sa letter head with his contact number and address confirming the name of the business and yung position ko sa company. tatangapin na ba yun ng vetassess or i-insist ko sa lawyer na lagyan nila ng address at contact number lahat ng documents na pina certify ko sa kanila? yung mga documents nyo ba na pina notarize naka indicate doon yung address and contact number ng nag certify?

2. nag email ako sa vetassess regarding sa paid evidence na kailangan ko i-submit. ang sagot nila is i should consider this as working on my own business and just submit either company taxation records, payslips etc. since maliit na business lang kami and my parents own it, wala ako payslip. i had the taxation records (percentage tax and itr) notarized it is under my fathers name kasi single proprietorship. i provided 1 record for 2003, 1 record for 2007 (under my fathers name) and 1 record for 2011 (under my name). would this be accepted as paid evidence? dapat ba ako mag submit 1 record for each year o pwede na yun?

3. last question, regarding sa statutory declaration, curious lang ako just in case kailanganin ko mag submit ng ganito. ang kailangan ba talaga mag witness is someone from australia whose occupation is on the sol 2? o parang affidavit lang din ito na kailangan lang ipa notarize?

hoping someone could help me with this concern, ready na kasi ako mag submit, gusto ko lang masigurado na tama ang documents ko kasi sayang naman kung hindi maging positive ang assessment.
