How to include a illegitimate child? PR 189

My friend have this case if you can help him out. He has a child born out of wedlock from his former gf. The gf did not mention my friend in B/C as a father instead leaving the father column as blank. Both of them got married. the gf has already kids but she did not change the birth cert of the child. The child uses the same surname her mother. Question, How can my friend include his illegitimate child in the application? What are the documents required?
Anybody knows this case?

Not sure lng on this...Nasa kanya ba ang bata? I think need nya i prove na sya ang father and the best way to prove it is sa B/C. He might need to the B/C with the consent of the mom... If under 18 yrs old ang bata and di married ang parents, merong consent form 1229 na need i submit. Both parents need to sign, not sure kung pwede na eto or need talaga iprovide pa B/C as proof.
