pero ang main thing na dapat ma-consider ng lahat ay sa passport, check ung two lines sa bottom ng bio page... walang middle name. kaya pag ni-scan siya, lalabas lang first at last names. so, with this basis, usually formal docs ko ay omitted ung middle dahil sa passport (wala kasi tayong national identification cards).

Kaninang madaling araw we're trying na maglodge. mukhang may problema ang cc namin ayaw magproceed. kaya wait pa namin yung bank. kala namin matatapos na kami kanina. hays.
Anyway po may question for clarification lang po:
a. sa ID Number - sa Form 80 kasi nilagay namin ang birth certi, sss, tin, philhealth at prc. pero nung naglolodge na kami. yung birth certi (using local registry no) and yung national government docs (PRC) lang nalagay namin. ayaw na magduplicate eh. pano po kaya ito?
b. yung sa Employment - ano nilagay niyo sa present? yung date ngayon? nagdirect validation ba sila sa SSS, PAGIBIG, BIR and Philhealth?
c. Education - Sa form 80 nilagay namin yung sa PRC since may nakaindicate sa form na qualification pero upon lodging wala naman. So di na namin nilagay? ok lang kaya ito?

a. Passport lang po nilagay ko. Wala na rin kasi akong ibang ID.
b. Pwede kayo mag-iwan ng partial date. So sa case ko, February 2016 lang nilagay ko nung naglodge ako. Nagupload lang po ako ng resume, CoE, and payslips (complete, monthly) as evidences of employment. Tinanggap naman po nila and di na humingi ng iba pang documents.
c. Yung university degree lang po nilagay ko upon lodging. I don't think required po ilagay ang sa PRC even sa Form 80 dahil ang pagkakaintindi ko po roon ay tinatanong po kayo about your educational attainment.
