OEC from POEA, again

For reference:
PDF 1: http://www.poea.gov.ph/services/workers/doc_namehire.pdf
PDF 2: http://www.poea.gov.ph/services/workers/NH_Professionals.pdf

1. Contract.

3. "Statement how you were able to get this employment, copy of Diploma, Employment
Certificates, Service Record or Training Certificates and License/Eligibility (PRC
card), if any." from PDF 1. Ano yung statement how I was able etc. ikukuwento ko? Hinihingi ba talaga 'to?

4. Medical - kailangan pa ba 'to given that nagpamedical na kami for the visa?

Yung repatriation yes, kelangan yun sa contract, in case di siya provided ng company mo, check with your insurance, most of 457 health insurance dito sa Australia covers repatriation, pwede mo na yun ipakita.

i don't have any idea sa iba, kasi wala naman ganyan samen nung nagprocess kami

    01 Feb 2016 - online application
02 Feb 2016 - received call from the HR
04 Feb 2016 - Skype interview (fail weak signal)
26 Feb 2016 - Skype interview (at their Manila Office)
    03 Mar 2016 - Offer received
    11 Mar 2016 - Contract received
22 Mar 2016 - Application for Nomination Lodge
    01 Apr 2016 - Nomination approved
01 Apr 2016 - Visa lodge with dependent (de facto)
    05 Apr 2016 - Medical (NHS - Makati)
    07 Apr 2016 - Medical result uploaded
13 Apr 2016 - Visa granted with dependent

Yung statement, short narrative lang according to POEA 7221144.

Re: medical and dependents, good to know, salamat ulit.

In case of serious illness or death kailangang may sasagot ng cost of repatriation (either insurance or employer).
