@coachella9 sa part T ko ilalagay, at least 3 duties lng ang ilalagay ko, yung sa SA application for nomination, dun ko kukunin yung ilalagay ko dyan sa Part T. Hindi pa ako ngStart talaga sa form 80 / 1221, basa2x muna talaga ako sa mga posts dito & mga blogs, share ko nga yung sa intended date of arrival, parehas ky @Hunter_08 may date pero the rest blank/none/ n/a. Kung ano mn mabasa ko sa ibang blogs, ipo-post ko rin kagaya nung proposed date. @MissOZdreamer kung anong date dun sa last page (declaration), same ilalagay mo dyan. Unless may ma-i-share yung iba bases on their experience. Positive lang. God bless.

@AIR Ok thanks. kaso pano kaya un kasi nagpa assess ako sa ACS nung April this year. based on their assessment, ang relevant sa employment ko is until April 2017 kasi ito ung assessment date ko. ok lang ba na ilagay ko sa form 80 and 1221 na ang currently employed ako which means end date of current employer is August 2017? thanks po

question po mga kapatid @coachella9 @Strader @AIR @Hunter_08 regarding sa medical forms ng Nationwide Makati na need ifill out, nakalagay kasi sa website nila ung 3 forms:

NHSA DEPARTMENTAL FORM (Permanent / Temporary Visa Applicants)
DOH EB FORM - for permanent visa 15 years old and above
HIV CONSENT FORM - for permanent visa 15 years old and above

since for visa 489 po ung sa kin, does it mean ung NHSA DEPARTMENTAL FORM lang fill out ko and no need na ung other 2 forms? Thanks po

Sa medical permanent tick mo dyan @MissOZdreamer . Skilled migrant kasi sa atin kaya lang provisional. Kung temporary ang piliin mo wala ata yang hiv test. Basta yon ang inexplain ng nationwide cebu sa akin.

Sa medical kasi walang difference sa 189 at 190 pareha ang required tests sa atin 489 as skilled migrant

by the way mga dear kabayans, sa pag fill-out ng form 80 siyempre focues tayo sa may sagot, pano naman sa mga not applicable? Did you leave it blank sa lahat or just wrote N/A?

Pasensya na kung nag appear Obsessive-Compulsive.Para naman ma fill-up natin ng maayos at di na tayo balikan if ever ng CO.

@coachella9 hello. it seems need nga lahat ng forms for 489 kasi nung nag generate ako ng HAP ID lumabas sa checklist ung HIV test as required which means same lang medical form ni 489 and 190/189 like what sis @Strader said

plan ko sana this week kaso may bagyo. sana maging ok na ang weather kasi manggagaling pa ko sa province. ang hassle din kasi pag umuulan. plan ko din kasi kumuha ng fingerprint sa makati police station after ng medical. ikaw ba?

Basta wag nyo lang kalimotan ang letter na generated sa my health declaration kasi yan ang ibibigay nyo sa clinic at yan din hahanapin nila. Actually lahat ng forms andyan na sa nationwide website download nyo nalang para mafill upan nyo na agad. Bigay nyo nalang pagdating dun at syempre yong passport nyo.

@coachella9 tama mga kabayan, basta hindi applicable, hindi dapat i-leave na blank. @MissOZdreamer ano yung sa fingerprint? yung police clearance ba yan? tnx a lot sa inyo. nawala yung signal ko. kelan po kayo mgpaMedical? di pa ako nkapagGenerate ng HAP ID. mukhang napag-iwanan na ako. @AIR madali lang kumuha ng HAP ID kabayan dahil ayos na ang immiaccount (finally!!! Ariel happened to me. haha) ang tagal nasira e. Pag kakuha mo, the next day pwede ka na mag pa medical if ready na.

Yes @MissOZdreamer yung date is same date sa declaration (last) page if currently employed & included sa pgClaim ng points. Remember dun sa EOI natin, left blank yung END_DATE if currently employed? This made sense. Nabasa ko lng rin to. I hope may mkaShare based on their experience talaga.

About filling out form 80, while wala pang sumasagot sa query on Q24, let's check this out.

@Hunter_08 past 3 or 4 years lang nire-retain nila online... kapag nag request ka ng copy within the last 7 yrs nasa $14 something... kapag more than that mas mahal.

@Hunter_08 ang alam ko mas maganda kung lahat, yung sa akin sinama ko lahat naitabi ko kasi hehehe... yung sa friend ko kulang cya so nagrequest cya IRAS, may grant na cya... so not sure kung nakatulong ba yun... mas safe na den siguro kung lahat ng years na claim mo meron ka.

@coachella9 @AIR thanks sa response. ung fingerprint para po sa SG police clearance. nag work kasi ako sa SG before for more than a year kaya need ko magprovide.

guys, pano kaya ung case ko. niclaim ko lang sa EOI ko for my current employer is until April 2017 kasi eto ung assessed ni ACS since eto din ung time na nagpa assess ako so hanggang dito lang ang claim ko. anong ilalagay ko sa form 80, until april 2017 ba? hindi ko madelete ung "current" sa first entry ng employment eh. tas sa form 1221 meron question are you currently employed? ang sagot ko is yes. nalilito ako sa ilalagay.hehe thanks po

@MissOZdreamer in my opinion, kung currently employed ka, you should indicate sa.form 80 na current. If inaalala mo eh yung sa eoi mo na assessment date is as of april 2017, there should be no problem kasi na ka lock na yung eoi for editing kasi na invite ka na, all you have to do is submit solid evidences like detailed coes, proof of paid salary showing you are currently employed ka the same company. Umuusad naman tlaga ang panahon and nagbabago at nadadagdagan ang work experience unless you resigned or have undertaken other work not related to your claimed occupation in your ACS assessment.
