  I think month and year yung hinihingi sa just try to remember the closest date.

  That will be valid po if it is asking since birth

  Yung form kasi hinihingi lang is the last 10 years
  For me i just backdated 10 years from the date you plan to lodge the application ,not exact date ,month lang like if i will lodge this Dec 2023,

  kung di kayo lumipat ng bahay, 10yrs lang to present. tama si @Ozdrims , from 2013-present (2023) lagay mo. kung kelan ka magsusubmit, yun na lang gawin mong basis.
