Form 1229 - Partner is not the parent of my child

Hello everyone. Just want to confirm lang po if may nakaexperience na rin and similar ang ginawa.

Here's our situation...
I am the main applicant and dependents are my husband and daughter (11y/o). Hubby is not the biological father. We used form 1229 sa visa documents ng daughter ko then nag attach ako ng notarized Affidavit of Sole Parental Custody.

Need po ba may pirma din ng biological father ung form 1229 or better po kaya na magsign kami ng joint statement na under my full custody si daughter?

Salamat po sa sasagot. 😊

If I remember correctly, the form simply asks for the particulars of the applicant and can be signed by any person.

Now, on the matter of paternity of your daughter, I would add a DSWD Clearance for Minor to Travel Abroad. That would be have more bearing than a simple joint statement with the biological father.

Sorry, I stand corrected... you do need the signature of your child's biological father on the form... I was thinking of Form 1221 and not 1229.

Thanks po @lecia @cucci. Next question, say pumayag na pipirma sa form 1229, any other supporting document (ID?? Thinking also of signing a joint affidavit.

@kb.rinoa yes yes stat declaration or affidavit is good enough.
