Hello po, yung mga may health undertaking regarding scar sa lungs, pa-share po ng experience regarding sa actual check up in oz? Ano po mga ginawa senyo? Matagal po ba yung appointment? Thanks!

Ang alam ko lang kapag may health undertaking mean matagalan na gamutan or may regular check up or may regular monitoring sa health. Madaming medical test at malamang every 6 month regular check up. Dipende sa request ng Doctor. Pupunta ka muna sa iyong GP then ang GP ang mag rerefer saiyo sa public hospital. Libre naman lahat sa public hospital kaya walang dapat alalahanin sa bills. cheers

May visa grant na kami last Feb 2014 pa, pero kasama din kasi ung Form 815 sa email. wala naman additional testing na ni require si CO pero dapat ko nga i comply yan requirements na nasa 815. Dito me sa Singapore nagpamedical and more than 10 years na healed ako 100% sure hehe. Plus naka entry na din kami sa Australia pro less than a week lang so no time for that. Pero pagbalik ko sa AUstralia next year asikasuhin ko din yan agad.

@aolsystems good day po tanong ko lang po same process lang po ba sa student visa ang health undertaking? Ako po kasi ang primary applicant at ang mister ko dependent ko po siya at may history siya ng tb 2005 pero ng 2005 dn po cure na siya, naka hold pa po kasi ang application namin dahil si embassy nag request ng addtional test sa mister ko ng sputum at culture at xray test pero tapos na si mister sa sputum niya at negative ang result, ngayon po wait nalang namin mag november para sa xray niya sa tingin nyo po ba pag tapos ng xray ni mister 2 or 3 days bibigyan nadn siya ng health undertaking? I believe and i think positively na bibigyan na kayo ng visa after na maclear siya na "hindi" naging active ulit ang tb niya. kasi usually kung talagang treated siya ng full 6months dapat pagxray ng St.Lukes ulit babasahin na ng pinaka head pulmonary nila sila magbibigay ng advise na walang naging changes mula at inactive mula nung huling xray. tapos after 1week siguro iinform na kayo or kayo mismo magfolowup sa st.lukes extension at incase ok na yun result at wala ng iba pagagawa si doctor yun head diyan ng pulmonary sa st.lukes.gagawin nila ipapadala na nila yun sa Australia Emabssy para idoublecheck nila kung tingin nila ok. medyo magwawait ka ng 1-2 months nun delay pero depende yan pero usually 6-10 weeks. pag nacheck na ng docto nila or tao nila taga recheck sa embassy ng Australia na tama yun diagnose ng head pumonary ng st.lukes which usually naman talagang pinapaniwalaan nila kasi mga specialist ito at magagaling na doctor ito na sila mismo nag assigned at kumuha para gumawa ng test. magpapadala sila ngayon ng 815 undertaking ngayon pipirmahan niyo yun saka niyo papadala sa CO.usually magugulat nalang kayo yun CO bigla magpapadala ng 815 health undertaking so pirmahan niyo agad scan tapos resend sa kanya agad. after mga 2-3 days siguro may balita na kayo at lahat kayo family magkakavisa na. Ang question ko pala kung ok lang ask ko Nagsmoke ba yun asawa mo? kaya ko kasi natanung malaking contributory factor yun kung bakit minsan kailangan nila gawin ulit yun mga test na yun.kasi may nakikita sila sa xray na nagsisismula ulit. or kaya lang nila siguro pinagawa kasi nga 2005 kaya naniguardo lang sila. pero no worries kung ok siya at talagang treated na siya dati wag kayo magaalala. nanigurado lang ang australia na dumating kayo na safe at di siya makakahawa sa mga citizen at mga tao dito.pero kung active namn siya sasabihin lang ng doctor kung kaialngan gamutin or kung ano man advise niya. pero basta fully healed siya wag kayo matakot. yun pagkakaroon ng scar or tb na treated hindi yan makakaapekto sa visa niyo. siyempre magkakaproblem lang talaga kung disiya talaga natreat ng full 6months nun 2005 malalaman din ng doctor yan. kaya pray kalang parati maayos din yan. ang impt, sa lahat maging healthy kayo. ang mahigpit ang australia talaga is kung may criminal conviction for the past 10 years yun talaga kaya nila mag deny ng visa at disiya i grant kasi ayaw nga nila may mga exconvict doon or mga kaso na convicted ng korte natin at mga hit sa nbi na hindi pa clear. saka since treated na siya wala siya dapat ipagalala.basta iwasan nalang mahawa ulit kasi sa pinas madali mahawa. kasi nga tropical country tayo kahit mga doctor nga may mga nahahawa ng tb. saka usok commute saka pag ubo ng katabi mo na di nagtatakip kaya lagi sila redalert sa pinas kasi mataas talaga kaso tb kaya dimo rin sila masisi na naghihigpit para lalo na di kumalat.dito rin yun husband mo check siya ulit para sure na di mag active pero dito libre na yun ang impt. lang makarating na kayo dito. thanks!

@vhoythoy ok sige.pero nakalagay doon within the first entry mo sa australia kaialngan mainform mo sila. tatawag kalang or mag login sa webiste na nakalagy sa form para mainform mo sila. yun 4 weeks is parang leeway mo yun na dapat inform mo sila. saka anytime na na lalabas ka bansa or magbabago address iinform mo sila?diba pumasok kayo australia as permanent residence?pag binasa mo yun health undertaking lahat ng nakasulat doon lalo na first page nakalagay yan doon.diko lang alam sa situasyon mo kung papano.kasi after mo inform sila makakarecived kana email tapos doon ka magrereply kung lalabas ka bansa or nasan address mo para ipapacheckup ka nila doon sa malapit ng titirhan mo. pakicheck mo kasi ako kadadating kolang ngayon August yun binasa ko undertaking isa isa word by word yun ang pagkakaalaam ko.pero siguro baka iba yun saiyo kaya yun masasabi ko yun sa akin form na binasa at finillupan. @aolsystems salamat po sa information, dito po kami base sa pinas, si mister po hindi po siya nag smoke pero ang alam niya nahawa siya sa dati niya kawork kaya nagka tb siya at cure na po siya nag gamot siya ng 6 months pero yung scar niya hindi na nawala sabi din ng doctor niya sa lung center hindi na daw mawawala yun importante daw magaling na si mister kaya yan din yung dahilan bakit nagpa additional test si embassy kay mister wait nalang po namin mag november para sa xray niya, salamat po sa information at god bless po.

Hi.Pa help po.Na required for further documents po ako.
Hiningan po ako ng transcrits, employment cert at bank cert. The bank cert dalawa, BDO with 10k USD (equivalent to 12500 AUD) and 1.2 M pesos sa PNB, all under my name.Ang na mention po ay ang BDO lang po.Walang sinabi na hindi valid ang 1.2 M na PNB.
In addition po, alam ko na send ko po yung transcripts at certificates. Triple checked po before binigay sa VFS agent.Confirmed too by the VFS na nasa pouch lahat documents.
Also, pina pa sign po ako ng HEALTH UNDERTAKING or form 815.
I am so confused..huhuh..please tulong...

Dont confuse Mag sign kana sa form 815. For formality na nag promise ka pagka dating mo sa Australia eh magpapa check up sa ka GP about sa heath mo at kung kinakailangan na i refer ka sa specialist. Dont worry yung iban GP kapag bulk billing wala kang babayaran. At kung nagpa check up ka hindi bulk billing malamang per consultation fee sa GP ay $40 to $80. Dont worry pag ni refer ka nman sa public hospital wala ka nman babayaran. cheers

Hi All,

My visa has been granted since last December 2015 and I have to enter OZ before May 2015 with an undertaking to contact the Health Undertaking Department within 4 weeks of arrival.

I am currently working in Singapore and just want to land in Australia to validate my visa as I am planning my BIG move to OZ next year.

I have following questions:

1. Is it mandatory to report to Health Undertaking Department on first short visit to Australia or it can be done later on second/third longer visits.
2. Will I need to stay in Australia until all the tests are done and reports received?
3. Can I take an appointment on email and plan my travel and accordingly.
4. If I cannot take an appointment via email then what is the process and time frame of getting appointments?

Or is it true that you don't need to get appointment for check up during validation trip. Once you permanently moved to OZ than only it is required to report health authorities within one month of your arrival.

1. I contacted Health Undertaking Dept on my second arrival since I only stayed 1 week during my initial entry. But I only had 1 month gap between my first and second entry.
2. I visited the clinic where I had my xray, and talked to the chest specialist about the xray result all in one visit. She told me that indeed the scar is still present but not indicative of TB. They will just send you an appointment letter after 6months for another xray again so you have to inform them for any change of address.
3. Yes, email them and they can book you for an appointment. It took 2 mos for them to get back to me and give me an appointment.
4. Once you received the appointment letter, call them to either confirm or find other available slots that can fit your schedule.

You will always need an appointment for services, even for a haircut or manicure. hope this helps.

Prior visiting the clinic, yung appointment mail ba containing information kung san ka magrereport for check up?

So possible kaya na I have to contact them on my first IED then give my friend's address kung san ako mag stay and HUS will send me appointment letter there about sa initial check up and necessary check up? Well I guess same question number 3. So meaning ikaw parin ba tatawag sa clinic. Yung clinic ba pede tumanggap ng call from offshore? So matagal tagal pala talaga no bago maka appointment

My plan kasi is to do the BIG on March 2016 pa. So baka pede yung ganong diskarte na email si HUS then visit nalang ang OZ pag kelangan mag pacheck up na.

@supertoblerone yes email them after arrival. Appointment letter contains what clinic your are refered too and that would be the nearest chest clinic from the address that you sent. As per experience, nung natanggap ko yung appointment letter, more than a month pa yung schedule na binigay. Safest basta do your obligation to email them. Kung magreply agad sila, try to ring them baka pwedeng maschedule ka ng mas maaga while you are in Melbourne

Pede kaya if may appointment ulet ako eh punta nalang ako oz to have the check up?

Thank you sa very helpful info.Na-send ko na po lahat ng kulang including the form 815 last Feb 13 but no reply yet.Sa nabasa ko po in the other posts at sa reply nyo, yung health undertaking ay ginagawa once im in Aus. But knowing na pwede pa ma-deny ang appplication ko, should I take the form 815 as somewhat a positive sign for a visa? Salamat sa mga info and God bless sir.
