Hi @aeshna here's the list of documents I attached to my application. Total of 39 documents all in all

Everything's just a colored scanned copy, except for the docs I submitted to ACS na ni-reuse ko lang rin for visa lodgement. Hope this helps!

1. Passport - biographical/photo page*
2. Birth Certificate*
3. eMedical Information Sheet
- Extracted mine when it was cleared
4. NBI Clearance
5. Form 80
6. Form 1221
- For both F80 and F1221, I filled them out on my computer, printed them, manually signed then scanned to PDF.
8. CV/Resume
- I made sure that the information here is the same as what I declared in my EOI, Immi Account, Form 80 and Form 1222.
9. PTE-A Test Results
- PDF copy of what I received via email
10. ACS Assessment Results/Certification
- PDF copy of what I received via email
11. Employment Reference (VOE)*
12. Employment Contracts
- I included all contracts I have on hand, and also included promotion letters
- Separate document per contract and promotion letter
13. Pay slips
- I submitted all that I have (almost complete, just missing a few months here & there), although may nakuha akong tips from others na okay lang kahit around 3 payslips per year, if yours is incomplete. Sinama ko na lang rin lahat ng meron ako to be safe.
- I compiled pay slips per year per document.
14. Tax returns/ITR
- Almost complete, 1 year was missing
- 1doc per ITR
15. Diploma*
16. English translation of diploma, since the original was in Latin*
17. Transcript of records*

*CTC; same doc I submitted to ACS

Here's the naming convention I used, baka maka-help rin to be more organized, although this is not required (except for the no spaces, no special characters rule):
Firstname_Lastname-Document_Type
example:
Czaaaps_Surname-ACS_Assessment
Czaaaps_Surname-PTE-A_Result

@aeshna Isa lang kasi company ko since after graduation until now, nagche-change roles lang throughout the years. I included all contracts for each of my roles. Except for one pala, kasi nagchange job title lang due to a reorg. I think it would be better if you can include everything you have, kahit siguro one from each company you've been employed to. Lalo na if you declared it and if you claimed points for that experience. Yung iba, they included SSS remittances, along with ITR and payslip. Di ko na naisama yung SSS ko kasi lagi nag-eerror yung website when I try to access.

For the ITR, I started work 2009 so naisama ko yun until 2016, except for 2010, di ko mahanap and wala na rin daw copy HR namin. Wala naman naging problem for the missing ITR.

@annemaersk: If na-OC ka (like me haha) since there's an attachment type na "Health, Evidence of", you can attach your "eMedical Information Sheet". This can be downloaded from your eMedical account. Naka-reflect dun that you have completed all examinations required, and also the clinic. Pero tama si @Santi, the CO should be able to see naman na cleared medicals mo since attached/connected naman both applications. Good luck!

ask ko lang...baka kse nagkamali ako ng basa, ung nasa checklist na lng ung nirerequest right? are there cases na nag ask pa sila ng more docs? or kung anu ung nasa list yun na lang kulang and the rest of the docs are ok na?
