    Hi everyone! Hope anyone with a similar case can answer.

    When I did my medical, I was asked to provide a surgery clearance for a cyst found during the exam. I had a prior clearance but it was dated 5 years ago so I was asked a more recent one.

    I provided the medical certificate as well as ultrasound results (recommendation was to monitor the cyst every 6mos to 1 yr) to Nationwide about a week or so after. My health assessment status changed to something like "will be further assessed by BUPA" so I assumed they passed the docs to them. A few days after, status changed to "no action required".

    My question is, do I need to upload these docs to Evidence of Health? I'm not sure hanggang saan nakikita ng CO, kasi kahit saan wala naman remarks na nanghingi sila ng additional docs.. For those with false positive TB test cases, naka indicate po ba yung result sa immiaccount?

    @audreamer05 hi! For us, hindi kami nag-upload. We tried getting CoE pero pahirapan ang pagkuha so we decided na wag nalang. Nadamay lang siya nung screenshot ng SSS pero other than that, wala na kaming ibang docs for unclaimed experience.

    Pero it depends din kasi sa CO, may nabasa akong may CO na naghanap so up to you, kung makakaprovide ka, go. :)

    @jazmyne18 thanks! Wag ko nalang din siguro muna i-upload yun COE until I get the bank statements from the bank. Prayers nalang siguro na mapunta sa CO na mabait at hindi na maghanap ng proof from those experience na hindi related.

    Hello all, niqquestion pa ba ng CO details ng birth certificate? Kung ung mother's surname (bottom leftmost) may typo isang letter. Kunwari instead na Alegre naging Alegro. Tapos ung surname ko is Alegre pati ng father sa document. Kelangan pa ba ipacorrect? And if meron kayo massuggest na contact or lawyer pa nga ata kelngan pag ganitong case ba? Thanks in advance.

    @ssendood if you're talking about your birth cert na mali yung nakalagay sa surename ng mom mo baka hindi naman din papansinin ng CO yun kung hindi naman sya kasama sa application mo but kung kasama sya ibang usapan yun.. pero syempre mas okay pa din na accurate yung mga docs na ipapasa natin.. just my thoughts.
