No need to notarized yung PTE/IELTS. Yes, submit mo din both form 80 and 1221. para makapag submit ka ng visa application need mo bayaran ng full amount then after mo mag submit dun ka pa lang allowed mag attach ng mga docs mo.

Yes. at need talaga yung form 80. makakapag upload ka lang naman ng visa kapag nakapag lodge ka na ng visa mo so kung may kulang or need kang ihabol na mga docs pwede kang mag attach basta hindi pa lagpas sa allowed numbers or attachment.
