You need at least 2 payment evidences from your old companies, how about ITR and SSS contribution copies? But for your recent employment you should provide your payslips and another payment evidence from the list. Good luck!

Sa mga nakita ko sa ibang applicants na may findings sa xray, hinihingian sila ng form 815 then visa grant afterwards if all other documents are okay. If okay lang sa inyo na mag hintay, darating at darating din yung grant nyo ng sabay.
