@Sygnoze medyo kumplikado nga. kung wala kang contacts sa kahit kaninong relatives ng biological mother mo ay mahihirapan ka matrace ang details na kailangan mo. May CO ka na ba? Baka pwede mo muna itry magpasa ng walang F80 baka magrant visa mo na di hingin yun.

i think you can leave it blank yung name ni father then explain mo dun sa last page yung reason why blank siya.. macrcross check din naman nila to sa birth certificate mo..

or yes, you can wait for a CO to be assigned..

@filipinacpa @engineer20 may CO na po ako..ung FORM 80 ung hiningi...cguro ung mother ko na lang sa birth certificate ang ilalagay ko. i will try to ask kung anong bday and place of birth nya..hihingi ba ang CO ng proof sa mga info na sinulat mo sa FORM 80?

at kapag hindi applicable NA lalagay or leave blank?
sa question kasi ng propose date of arrival and what state wala pa akong definite na answer kasi..any idea?

@Sygnoze i think the purpose of form80 is parang self declaration of everything for them to get to know you more.. so i think yes u can do that but i suggest explain it as well dun sa last page where were are given the chance to discuss anything vague on our answers sa form80

i submitted form 80 too and did not provide any other evidences to support it

@jlexy sa travel isusulat ko lahat. sa aming magasawa aabot ng tig-30 trips. nagtabulate pa ako sa excel para madaling masort ang dates.

hala kaloka andami nito. past 10 years, so kasali po expired passport?

yung travels po sa expired passport need rin isama sa Form 80.

@se29m no need ang visa stamps.. you don't need to attach other docs on your form 80. just fill up the form 80 alone.. yung passport bio page naman is already uploaded na sa other section ng mga attachments.. so form 80 lang.

Print hardcopies then manually fill-in the form then scan pa din ang paraan now sa form80? Or pwede sa form na mismo electronic fill-in?

@IslanderndCity pwede na sa form electronic fill-in.. pero parang mas naging convenience sakin yung print all pages and write the answers nalang manually.. hehe..

Anyway, sa mga taga-Singapore na need ng hardcopies, I have 3 sets. In case hindi accessible sa office nyo ang free printing

Hello po ask ko sa part ng may relatives info dito sa Form 80. I have a half brother and sister na nakatira sa Mt. DRuitt Blacktown sa Australia. I have their address and my sister got married I think mga 3 years ago. Question ko po ay do I need to include the name of her spouse and their children kasi iba na yung surname nya. At do I also need to include the name of my stepmother and her husband sa form 80? salamat po in advance:)

tanong po sa FORM 80.. sa address history, may bahay kmi sa pinas pero tuwing bakasyon lng ang uwi, pano po input ang date from and to?
