By the way, may nakatry na po dito na SELF statutory declaration ginawa to declare sa job description as HR is providing only generic coe? And para hindi na rin kailangang sabihan pa si boss? This is for engineers Australia skills assessment po.

I am guessing you are confused with a Stat declaration ..... yung stat declaration po e para ka lang ng nag kkwento ng totoong nangyari at parang cinecertify mo na totoo ito witness by a ( JP dito ) kung nasa pinas ka ipanotarze mo lang ...

pwede nyo pong pagsamahin ang COE at ang stat declaration

what we did e gumawa kami ng sarili naming template ng COE + stat declaration ( ACS kami ) the have it print with HR or boss para with letter head .. you have no choice na inde mo ipaalam sa mga boss mo or immediate supervisor mo kasi malalaman at malalaman nila yun .. pwede kasi silang tawagan to verify your work details .. kung ayaw mo sa boss mo humanap ka ng ka-close mo na senior na pwede mag testify ng work mo .. just have this sign and notarized and you are good to go

by the way inde ikaw ang gumagawa ng declaration kung inde yung taong mag sisign ng COE + stat dec mo

I dont know if this is true kasi yung Bayaw kong civil engineer ni require ng Agent nya na humingi ng COE at kailangan i sign immediate supervisor ....

since ayaw i since ng boss nya inde na umuusad yung application nyan ... well anyway good luck to your application

What if the company is not existing anymore? I have a contact with my employer kaya lang nasa malayo sya hindi sya makapirma sa stat dec at coe?

@JCsantos, Sir hingi po ako ng advice sa inyo dito po kasi ako sa Qatar ngayon. my coe na ako with letterhead un nga lang generic. Ang nag sign ung mismong owner ng company, infact nag alibi lng ako sa kanya kc nagtanong sya kung saan ko gagamitin. about naman sa job description ko kinausap ko ung visor ko if pede sya mag issue ng cert. of job description ko since sya naman ung nag momonitor at nag aasign ng trabaho sa amin but then nireject nya kausapin ko daw ung owner na pumirma ng coe ko but, I doubt cgurado rereject nya din (owner of the company) ang request ko. posible ba na e-accept ng vetasses if ung colleague ko ang magtestify para sa akin? parehas lng posiyon namin , pareho lang din ung ginagawa namin araw-araw at same day and same year din kami nag start na magtrabaho sa kumpanya na pinapasukan ko nagyon. Im not telling you to do this .. you can make it appear that your mate is more SENIOR than you .. then he can attest to your job description ....

@JCsantos,Sir maraming salamat sa advice. siguro mag self statutory na lang ako nabasa ko na ina accept naman daw ng vetassess ang self statutory isama ko na lang sa attachement ung contract at hingi na lng ako ng bank statement.

This is my first post in PinoyAU and I find this site really informative specially for Filipinos who like to go to Australia!

I'm now writing a Statutory Declaration (my company would not give detailed Employee Certificate) for ACS assessment and I have few questions/concerns that I'd like to clarify.

1. Can we use the Statutory Declaration or should we use the Affidavit instead?
2. If we use Statutory Declaration, can this be notarized in Philippines?
3. and lastly, does it need to have a "Before me" statement because I cannot bring my colleague to the notary public for notarization?

A. Mr. X Y Z was a Full Time Employee of ABC company from start to end.

Note that yung date kong office mate (Juan dela Cruz) ang gagawa nitong Stat Dec para sakin and wala na rin sya sa ABC company. Tapos same kami ng position date kase no where to find narin ung mga dating visor and managers namin (karamihan nag abroad na).
Bukod sa Passport ni Juan dela Cruz, ano pa bang possible na isama ko? Kailangan bang ibang ID's or COE din ni Juan dela Cruz from ABC company? Or ano pa kaya?

@alexamae ako nagpass ng statutory declaration ko, containing my position sa company, mga tasks, at explanation kung bakit di na makakakuha ng COE sa company.
nagpass din ako ng statutory declaration of a witness galing sa former colleague ko. about yun sa profile nya, kung ano job namin sa company, sinasabi dun inaattest nya yung skills ko.
mas ok kung galing sa boss mo kung may contact ka pa sa kanya at willing sya pumirma sa statutory declaration.
pinagawa ko yun sa lawyer then pina-notarize.

hi @risa_c, naglagay ka ba ng payslip from that company? nadissolved na kasi yung company ko dati. nagbigay ako ng statutory declaration.

question lang po. nag-upload ako ng Statutory Declaration for my past work experience kasi nagsara na yung company. nag-email back ang ACS sa akin. sabi nila, kelangan daw ng either payslip or termination letter from the company. di na ako makakakuha nun.
naka-attach lang ang company ID ko dun. inexplain ko na lang.


sa tingin nyo matatanggap ito ng ACS?

This should be Ok kasi based sa timeline mo, suitable for Bachelor's degree and credited ang work experiences mo eh. Congrats!

No problem po. So that other people here would know also na na-approve ng EA ang application ko kahit iyong 1 document ko ay walang pirma ng boss ko.

Di nyo po pinasign yung senior colleague or Head nyo? Kasi sa case ko di nagbibigay ng COE w/ detailed JD dahil di pa daw tapos gawin ng boss namin kasi nag reorg kami.

Hi...need ur advice.Kelangan ba gumawa ng stat declaration/affidavit kung walang sss contribution yung previous company na pinag ttrabahoan ko?bali nagwork ako march 2005-april 2008..pero from may 2007 lng nagsimula yung remitance....gamitin ko sana yung Sss contribution as proof of paid employment..please help

@chu_se there are other options naman such as ITR and bank statement that can be use as proof of paid employment

I only have payslip..and need atleast 2 proofs...walang bank statement,itrs..since ang tgal n nuon...

@islanderCity, thanks for the reply.TRA requires that all docs they recieve should not be original and have a ctc stamped by authorized body. So kailngn din po ba naka CTC yong ipadalang stat dec? Thanks po. I mean notarized na kasi

@Megger Yes, the same lawyer.
The scenario will be like this:
- You will make your Statutory Declaration (that's why it's a.k.a. self-stat declaration)
- Your lawyer will notarize it but of course with supporting documents like your payslips, Company ID, work contract (whatever is available to help "prove" you're indeed working there as Supervisor/Engineer/Technician whatsoever in that particular employment period)
- Your lawyer will notarize your stat dec. Your lawyer will CTC only those copies of your company ID, etc.
- Your lawyer will state as an Appendix of the Stat Dec like this, "These are the Supporting Documents referred to in the Statutory Declaration of Megger Santiago-Duterte, blah blah blah, declared on 25th of October 2015, collectively.
